The child is a 16-month-old who has been receiving their food/nutrition through a feeding tube. The child was recently approved to switch to receiving food by mouth. Therefore, the family is in need of a highchair to help assist during feeding times.

Client has a severely autistic child who is now 5 years of age plus a 2 year old. Client has worked off and on and she had been working from September until March and then had to quit because of the 5year old behavior due to his autism. She recently moved because she thought it would be better if she was closer to her mother and sister. Her mother is who she can count on for helping her with the kids. FOB has not been a resource and her mother helps her when she can financially and always emotionally and physically her mother helps her with childcare, respite care etc. Client is requesting help with hygiene products (deodorant, lotion, female hygiene products), dish soap, Scott towels, toilet paper, pull ups/diapers and wipes for the 5 year old as he is not capable of being potty trained, laundry products and cleaning supplies for her home.

This family is needing assistance with rent/deposit for a new home due to the family needing more space for a child they are taking as kinship placement after a life altering accident. The child needs more space for the medical equipment along with the ability for them to work with nursing, physical, and occupational therapist in the home. Assisting with the rent/deposit will allow the family to move and settle in to their new home, and allow the child to discharge from the hospital after being admitted for over 6 months. Any assistance is appreciated.

Four children have been placed in a kinship home and as part of their preparedness kit, required by licensing, they all need to have sleeping bags in the event of an emergency. The children are bonded to this placement and even though she is not blood related, then think of her as their grandmother. Getting 5 sleeping bags would be greatly appreciated, as this is an expense the parent can't afford at this time.

The purpose of this request is to assist a youth as they prepare for the next chapter. The youth is preparing for college and has created a wish list through Amazon. Upon the request meeting approval, case manager will forward Amazon wish list link of what youth is requesting to assure they have what they need for their first year as a college student. A devoted mother has expressed her need for a one-seater stroller suitable for her infant. As a single parent, she is navigating the challenges of providing for her new child independently. When we inquired about how we could assist her, she shared that having a stroller would make a significant difference in her daily life.

During the home walkthrough, it became evident that this family, with four wonderful children sharing a bunk bed, would benefit immensely from having individual beds for each child. We strongly advocate for providing a bunk bed with mattresses for this family, ensuring that every child has their own space to rest and dream. Thank you for your support for this family.

Hello, I am requesting a $200.00 gift card for a sixteen-year-old young man who resides in a Department of Mental Health Residential Home. He is diagnosed with Autism and uses a communicative device to communicate with others. He enjoys bowling, basketball, going to Get Air Trampoline Park and shopping at Red Rocks Thrift Store. This money will positively impact this young mans life by ensuring that he can spend his summer participating in some of the activities he enjoys.

The family has no immediate supports for assistance with obtaining furniture for the new home. Family is struggling with stability and mom's mental health.
