Help Improve a Child's Safety/Wellbeing

The Department is tasked with providing youth with a stroller for his one-year-old child.The youth has expressed the need for a three wheel stroller so he can take his child out of the home.Over the past two months, the youth has continued to display growth and maturity as he is in full compliance with probation, delinquency court, followed placement rules and remained in care. The youth continues to make strides in his academic success as he attends school weekly. Outside of school, The youth began working with a friend working construction Monday through Friday from 7:00am-3:30pm. The youth is an active father visiting with his 1 year old son several days throughout the week as well as occasional overnight visits. The youth maintains his son is a huge influence and motivation for him to continue to do well in order to lead by an example and be a consistent father. The Department has identified the following strengths: the youth is open and receptive to professional support, has insight into life problems and trauma, and how they affect their daily life. The youth also has the ability to self-advocate. The youth’s goals are to remain in care, graduate high school and work/fulltime so that he can provide for his son. For these reasons, I am advocating for youth to have the need met in order to provide additional support resources for to the youth. Minor is graduating middle school and has been working really hard to meet the requirements. She has had major setbacks this year, with the death of a sibling, the incarceration of her mom, and the absence of her father. Minor is in foster care, and it has been a tremendous adjustment given her severe trauma. She has worked very hard to makeup work, improve her grades and was finally informed she will be culminating this month. This is truly a big deal for this youth, she has faced several obstacles including having an IEP, being a Regional Center client, and stabilizing her mental health. Asking for a spa day for two to Bliss Nails and Spa in Lancaster, closest to her home. The full spa nails experience is about 120 per person. This is truly a big deal for this youth. Would like for the youth to feel like a normal teenager for a day.

My client is a single 18 years old mother, she has a 2 year old child who resides with her in a SILP setting (supervised independent living placement). My client still attends High School and does not have a job because her goal is to complete High School. My client does get SILP funding but uses the funding she receives to pay for her SILP and to purchase items of need for her child. Most recently my clients refrigerator stopped working and she does not have the money to replace the refrigerator. My client would be most appreciative if she could be matched to a sponsor willing to purchase a replacement refrigerator for her. My client is in need of a working refrigerator to store her monthly groceries. Not having a working refrigerator creates a financial burden for my client because she does not have money to order out daily and cannot keep her groceries from getting spoiled.

The family has rented an apartment, but they do not have sufficient beds or furniture for the family. The three year old is in need of a twin bed/mattress. Help Preserve a Kinship Placement

This family is composed of a husband 60 and wife 58 They have children with severe special needs, a son bedbounded age 29, a son aged 25 and 22 in college and raising three boys ages 17-18-19 who have Autism syndrome who under DCFS FosterCare placement under NUFARM they are so committed to all the Boys and filling their care.
One of the boys is over 320 pounds and the other is over 200 pounds they share rooms they sleepin single Bunk Beds and need to be replaced with double bunk beds. She said with me being a stay home mom and caring for all the kids. They have not been able to replace the beds at this moment so they are asking for a grant to provide the bunk beds and Mattresses.

The mother and children need groceries and basic food items to provide them with nutritious meals for the month. The mother moved she and the children to a new home where rent is now higher and as a single mother without government assistance, she has not been successful in meeting all needs with her employment salary. Mother case recently opened and per her case plan goals, she enrolled in individual counseling. Furthermore, caregiver is committed to maintaining the family together. The main reason I am requesting this need to be fulfilled is to ensure safe environment for the children. DCFS is requesting improve parental capacity and enhancement of the mother to provide the children permanency, to foster emotional well-being. The children need to have a home with sufficient food to ensuring that the have a healthy environment. DCFS has assisted the family by providing referrals to programs. DCFS will continue to link the family to community services such as food banks, counseling, etc. in order to ensure the family continues in maintenance. The estimated cost of the need/s being requested is $400 worth of food market.

66 Yr. Single relative caregiver with one household income has become the caregiver of her 15-year-old nephew, this has had a significant positive impact on the minor. This is because it provides a sense of stability and continuity, preserves family bonds, and can reduce trauma associated with foster care. This caregiver and nephew need your support with their electricity bill. Prior to this relative caregiver caring for nephew, she had two full-time jobs to make ends meet, now that she is taking the responsibility to care for him, she had to scale back to only one job to ensure she has time to care for her nephew and keep this teenager safe, busy with extra curriculum activities. The caregiver’s sustainability plan is to carefully manage her income and expenses and build a savings fund to cover unexpected costs that may arise.

Help Support a Youth Aging Out

Non-Minor Dependent needs $825 to fulfill her rent payment for June. Non-Minor Dependent splits her rent with her brother, however her brother has not been able to work this past month due to a leg injury. Therefore, the Non-Minor Dependent needs to pay the whole amount for rent. Although, Non Minor Dependent is having that challenge, non minor dependent is meeting the case plan goals/court orders and is currently working as a CNA. Furthermore, Non Minor Dependent is committed to learning how to live independently and prepare for the future. During my work with Non Minor Dependent , I have identified the following strengths: hardworking, caring, and loyal. Non minor dependent goals are to keep saving and apply to a LVN program. The main reason I am requesting this need to be fulfilled is for Non-Minor Dependent to maintain her housing and avoid any legal or financial penalties. This need is critical to ensuring that Non-Minor Dependent has her basic needs met of having a safe home. DCFS has assisted the family by providing financial assistance with transportation to work, a monthly stipend, and work clothing allowance. DCFS will continue to link the family to community services such as low-income utility assistance, money management, etc. in order to ensure Non- Minor Dependent successfully prepares for adulthood. Help Preserve a Kinship Placement

I am advocating on behalf of a relative caregiver who cares for her niece and two children on her own with no outside support. Although caregiver was able to escape a DV relationship last year, she finds it difficult to navigate day-to-day life after the death of her sister and being a now single parent. She has recently obtained child care for her niece so that she can go back to work as an IHSS caregiver to better support her family and provide for their needs. Caregiver just needs a stepping stone right now to help her and her family get to where they need to be. Therefore, I kindly ask for assistance with a gift card for school clothing and shoes for the kids.
