I have a 18 year old youth who just moved into her own apartment. Youth is in need to household essentials and furniture. Youth would like assistance with a queen size bed, frame and mattress, dresser, and hangers. As well as kitchen and bathroom items. Youth would help with a broom dustpan, mop, mop bucket, cleaning supplies, laundry detergent, couch, chair, and lamp.

An aunt is placement for her 3-month-old niece. She is requesting help with formula. The infant takes Nutramigen Hypoallergenic Ready-to-Feed.

A grandmother has recently taken in her 2 year old grandson. The grandmother is in the process of filing guardianship. The grandmother is looking for assistance to help care for the child. The grandmother is seeking the following:

Diapers: size 4/5 (child is 35-40lbs) prefers the LUVS brand Wipes: sensitive skin Clothes(preferably summer clothes): 3T

Mother reached out to advocate requesting help with a washer & dryer. She is a single mother of children ages 11, 13, 14, 15 and 17 along with two adult children. She was renting these appliances from a company and could not keep up and they removed them from the home. This request would help ensure the children have clean clothing and improve their hygiene and wellbeing.

Single Father needing gas card until next paycheck and groceries. Get paid in next two weeks but has limited food in the fridge to make meals in the home. Will be able to maintain providing moving forward.

A 17-year-old female was recently placed with her 22-year-old sister. Both young ladies have been in the child welfare system for many years, and the older sister has successfully completed the Collaborative Care program. She now works full-time, has her own home, and is raising her own young child.

Placement is happy to have her sister in her home and has been proactive with ensuring her sister is getting all the services and assistance she needs as she begins transitioning into adulthood. However, her own income is limited, and she is unable to purchase a bed needed for her sister. Having a bed would ensure that the younger sister has a safe and appropriate sleeping area, enabling her to stay in placement with her sister. The younger sister (ward) also struggles with bed wetting, which is being addressed medically and therapeutically. Mattress covers and bedding are also needed.

The family consists of a Father and two teen children. The family is low income and does not have much food in the home. The family has some nonperishable items but not much food in the fridge or freezer. Father reported they could use some meat so he can make meals for the family or items that the children are able to prepare for themselves when he is working. Father would appreciate any help he could get at this time.

I am requesting a twin be and bedding for a child placed in a relative home. They have income but they have their own child and expecting second child. The child placed in their home using one of their step child's bed. In summer he comes to visit his father. He came two days ago and using his bed. The child placed in the home needs a bed. Without a proper bed the family will not able to meet basic living standard for her. The family is committed to create a safe and stable environment, and receiving this assistance would greatly support preserve the placement.

Mother and female child (3) are in need of summer clothes. Mom is a full time college student and works full time from home usually until 8 pm Monday-Friday.
The 3 year old wears 4T/5T in clothes and she loves Minnie Mouse and colorful clothes. Mother is looking for any work out wear for the gym. She is in a XL and women’s underwear in XL and socks.

Please note that underwear and socks should be new.

I am requesting a crib and a crib mattress for a Relative Resource home, in order to comply with the requirements for safe sleep and to preserve the placement in the resource home. The child is placed in a limited income home. The resource parent receives a disability check monthly and currently unable to afford a safe crib. Having a crib is essential not only for child's safety and well being but also to meet the standard of the program. The family continue committed to provide the child a safe and nurture home.

A single mother recently got her car stolen and several items. Mother no longer has a car seat for her 4 year old child (36lbs). Mother is seeking assistance for a car seat while navigating recovering other items.

A mom is working to reunify with her 4 children. In order to do so this mom needs to be able to participate in Visitation with the children. Mom needs assistance with gas in order to be able to travel back and forth to visits. Mom is having to have 30-40 minutes each way to get to visits with her children. Mom has been working hard to complete all other services for DCS but needs assistance so that she can get to visits.
