Maternal Grandmother is requesting assistance to get the child diapers size 4 and wipes. Maternal Grandmother is not receiving monthly reimbursements. Maternal Grandmother is paying for daycare out of pocket.

The family would benefit from new mattresses as they are currently using dirty mattresses and is unsanitary for the children. Replacing these items would help improve the children's sleeping conditions and overall, wellbeing in the home.

I am requesting financial assistance for the client I am currently serving who is a single parent to five children. She has five children ranging from the ages from 8 to 14 years old. Mother just obtained a new job but was unemployed for two weeks causing financial hardship. Mother's monthly rent is $500 dollars but I am requesting $300 dollars to alleviate financial stress for the month. I am also requesting a HEB gift card to address food needs in the home due to having minimal income.

Mother of 2 children (17-year-old female who has 2 children of her own and is pregnant with her third, and 12-year-old male) are in need of financial assistance with either groceries or utilities. Mother is currently not employed however is seeking employment.

Mother of 6 children (8-year-old female, 8-year-old female (TWINS), 6 year old male, 6 year old female (TWINS), 3 year old male, and 2 year old female) are in need of assistance of paying her light bill, mother is currently separated and is not employed. Mother is seeking a job at this time.
