The child is currently placed out of home. The mother is going to be on a case plan and needs to demonstrate her stability. Her car is not working and this is impeding her stability as she does not have reliable transportation for her child.
It is unsure excatly what is wrong with the mother's care. The mother stated she may need a new intake manifold, head gasket, radiator and her back window replaced.
I am not sure what the estimated value is.
The family needs help cleaning the inside and outside of their home. The father's paramour recently passed away and the father is struggling to maintain the household.
There are big items that need to be removed and thrown away - I am requesting that the trash is able to be taken to a dump as the father was fined a large sum of money from having large items in the trash. He is currently unemployed and cannot work due to medical issues.
Due to the father not working, he is having a hard time making ends meet and his utility bill is due soon.
The child suffers from a skin condition and the mattress is unsalvageable. It is believed that a new mattress with help ease his skin condition and prevent flare ups.
The caregivers have two children placed with them now. One is an infant and one is a 14 year old boy. The caregivers are expecting a child of there on any day. The caregivers are in need of bed for the 14 year old child. The child is large and can't comfortably sleep on a twin size bed. The child is 6'2" and weighs 200lbs. The family is moving to a larger home to accommodate the children. At minimum the child needs a queen size bed so that he can get the proper sleep and care that he needs.

Currently three girls age, 4, 5 an 14 are sharing a room and bunk beds. Their bunk beds broke and are in need of a new set. They were reunified with their mother this May. The mother just had her fourth child and will be coming home from the hospital soon. The mother is a substitute teacher and will return to work in a couple months when she is able to. Their refrigerator is very old and acting like it is going to stop working soon. Also, their electricity bill continues to rise and they are struggling to pay the $400 this month.

The mother needs help moving her belongings out of an abusive household and into a home where she and her children will be safe.
This will help the mother during a stressful and overwhelming time and prevent her children from being in an unsafe situation. It was also prevent the mother from being a victim of abuse.
I have a family with six children in our care who are in a bind. After some miscommunication between the grandmother, the landlord, and Section 8, the family owes the landlord $3600 for two months unpaid rent. The grandmother has been consistent with her rent payments for two years now, and was planning to move from her current house that she is renting. However, after those plans fell through, she was forced to remain in the house that she was trying to leave. However, two months rent had gone unpaid, and now the landlord is threatening to evict the grandmother and the four children. The grandmother can afford and handle her future rent payments, but the $3600 past due she cannot afford, and Section 8 has said that they cannot help with this. It would be a substantial help if any finaicial assistance can be provided to the family in order to help chip away at the $3600 owed.

Client is a single mother of a 13-year-old girl. The client and her daughter were staying with her cousin, but the cousin told her she had to be out by today 08/01/2022. The client has nowhere else to go and only has enough money to cover one night at a hotel. After that, she will be homeless. The client has not been able to find an apartment that is within her budget at this time and is still looking diligently to find something stable for her and her daughter. The client starts a new job on 08/08/2022 and is in need of assistance with the hotel payment until she starts getting paid.

Client is a single mother of 3. She recently gave birth to her 3rd child on 07/29/2022 and is currently at home recovering. As a result of pregnancy complications, she had to leave her job. Her utility bill is due soon and she doesn't have the funds to pay it. Once her 6-week recovery time is over she will find new employment as well as try to get her previous job back if possible. Please help this single mother keep her electricity connected so she can care for her newborn baby as well as her other 2 children.
Mother is in need of a cell phone so that providers and case management can maintain contact. Mother is a single mother with six children ages ranging from 17yrs - 1year. She is not working and due to not having a phone it is hard for medical providers, school officials and social service providers to assist and help her maintain custody of her children and keep appointments. Having a phone to rectify lots of stress and put the children and mother in a better situation and allow the mother to attend court by Zoom and to complete her recommended case plan tasks. The department has a case with a single mother of 4 kids. The mother is currently struggling to make ends meet at this time. The mother can’t afford school clothing /supplies for the children for the upcoming school year. The mother has stressed for assistance as the kids have outgrown and or messed up most of their clothes/shoes. These children are in need of back-to-school Clothing and supplies they are all in Elementary school (No uniform). The mother works hard to provide for her children and is the sole provider of the family. Any assistance would be greatly appreciated for this family to start the school year off positively.

A one year old baby and her mother is in need of financial assistance with daycare fees. The mother recently lost her job due to having to leave work early to pick up her child whom was sick. The mother is currently seeking new employment but needs assistance to be able to have the time to do so. Having the child in daycare would allow the mother to fill out applications and take on interviews so she can get that steady income back. If the daycare fees are not obtained, the child may lose her spot at her current daycare. Any assistance would be greatly appreciated by the mother.

Good Afternoon, I'm currently working with a Mother with 6 small children with the ages of 10, 7, 5, 4, 2 and 1 year old, Participant was referral to the Hope Florida Program - Care Navigation, the family was living temporarily from place to place and on July 11, 2022, they will move into the rental home they have finally located. This family requires assistance with the costs of relocation (First and Security Deposit, Total of $2,500.00). Anything given for this Mother and her 6 children will be greatly appreciated. A 4-year-old child is in need of extensive dental work due to severe decay in her teeth. In order for the procedure to be done the child will need to be put to sleep due to the amount of work that needs to be done. The child has Medicaid, however, it does not cover the cost of the anesthesiologist which is approximately $200. The appointment can not be made until the anesthesiologist fee is paid. The family is unable to pay for the procedure which is why it has not been taken care of sooner.
