This young mother is now enrolled in American Career College to become a medical assistant, and is on leave for three months after giving birth to her first baby girl in September 2023. The mother will resume school in three months and has submitted the required documents for school to justify leave. This client is being submitted for services/support/referrals in meeting her dental needs as she did not get braces prior to the age of 18 years old. She is willing to use her existing foster care checks to make payments, therefore she is requesting assitance in locating community resources for reduced orthodontic care for foster youth. She is also a new mother and has requested a playmat for her young baby who will benefit from a stimulating mat. This birth father has worked hard to reunify with his two very young children. He successfully completed all required case plan activities and is ready to be a single father. The court agreed with this and placed the children back into his care full time this week. The father's youngest child (boy) is 1 1/2 years old and is special needs, which requires multiple ongoing medical appointments and therapy. The father will need a car seat to assist with transportation to and from these appointments. Therefore an infant car seat is being requested on his behalf. The father is very grateful and has declined any other needs as he was able to get a car seat for his daughter (age 2 1/2) and a double stroller for them. He is very humble and knows he has to be there for his young children since they have been in foster care for over a year. Caregiver/NMD needs a refrigerator. Caregiver has been trying to save up for one but has not been successful. Although, caregiver is having that challenge, caregiver is trying to her hardest to provide for her children. Furthermore, caregiver is committed to maintaining the family together. During my work with caregiver I have identified the following strengths: mother works, is knowledgeable of child’s needs and is present.. Caregivers goals are to be able to live on her own. For these reasons, I am advocating for caregiver to have the need identified met. The main reason I am requesting this need to be fulfilled is because mother is having a hard time to work and then stop by the market and cook fresh everyday for the 3 children and herself everyday. Mother and children were kicked out of her old apartment and was not allowed to remove anything from the location. Mother lost all her appliances and everything for her kitchen. Mother stated losing everything was hard but the worse thing is not having a fridge to save food. Food has been spoiling if they don’t eat it right away. Mother does not get CAlfresh and is buying her food by her hardworking money. This need is critical to ensuring that family can stay fed and save money. DCFS has assisted the family by providing a referral for kitchen items to Jamesstore. DCFS will continue to link the family to community services such as food banks, counseling, etc. in order to ensure the family continues in maintenance.
