Our clients (10 and 6) and foster mom were recently involved in a tragic car accident. The collision completely destroyed their car. Fortunately, our clients suffered minior injuries. However, foster mom is scheduled for surgery on her arm. Foster mom is the only caregiver in the home as her husband past away from COVID. Our clients lived through his passing and were definitely traumatized after this accident as well. During my last home visit, they shared they've been helping foster mom with cleaning and "cooking" sandwiches. Their bond is visually beautiful! Family is eagerly waiting for their adoption date. Our clients and foster mom would benefit from giftcards to grocery or clothing stores to help with expenses through this hard time.

This family has been compliant with Family Preservation case since May 2023. They were recently reunited after being separated for 18-months. During their separation, mother’s income lowered. Therefore, she did not have sufficient funds to purchase needed items for the home in preparation for her children’s return. With the help of family and friends she received a used refrigerator, which was in working condition at the time it was installed. However, it was recently brought to case worker's attention that both the freezer and bottom sections are no longer working to keep food at safe temperatures. Case worker is unable to submit a request for assistance to DCFS, as it is the end of the fiscal year, and new requests will not be accepted until July. I identified the best priced item that can be provided to the family and placed the website to it within the notes section, although items from other sources are welcomed.

A single mother is in need of support with obtaining kitchenware items (cups, plates/bowls, cooking and eating utensils, a pot/pan set), a queen sized bedframe w/ assembly service, and a queen sized mattress for herself. The family just recently exited homelessness after several months. The mother just now obtained part-time employment that affords to sustain the family ongoing. The children are in need of these items to have home cooked meals accessible and the mother currently has no bed, although the children do have beds. The family is closely bonded and has been participating in therapeutic services, parenting classes and other community based services. Any assistance would be greatly appreciated. I identified items from Walmart (most convenient priced vendor) that can be provided to the family and placed the website to the registry within the notes section, although items from other sources are welcomed.

She is on the Track team and is in need to track shoes called Nike Zoom Rival S 9 Track and Field Shoes. The youth wears a size 9 in womens. Mother is in need of the following household items: 2 dressers and 2 window gates Mother is a single mother of 3 and has limited income. Although, mother is having this challenge, she has been meeting the case plan goals/court orders, has completed most of her programs and is working to have her case successfully close. During my work with mother I have identified the following strengths: ability to cope with stress, motivated to succeed, resourceful committed to her family. Mother’s goals are to continue working to provide for her family, obtain a second job due to financial stress and start her own business. For these reasons, I am advocating for mother to have the needs identified met. The main reason I am requesting this need to be fulfilled is because in order to provide a safe, clean, healthy and comfortable environment for her family. It is essential to have these items as mother is having to eat out often, the house can be better organized and safer for the children. DCFS has assisted the family by providing rent assistance to the family. DCFS will continue to link the family to community services such as food banks, counseling, etc. in order to ensure the family continues in maintenance. For other items needed, I will explore other DFCS options for funding and in-kind donations. This need is being requested through Care Portal because unfortunately DCFS does not provide these items. There's a family I've been working with this year that is low income.  The student I work with has a brother who is special needs and bound to a wheel chair.  Mother is a full time caregiver for her special needs son. The two students receive counseling services to assist with various family dynamics challenges and adjustment challenges in middle school.  My student has been having difficulty with peer relationships due to low self-esteem.  He has expressed interest in working on his health over the summer and asked mom if she could get him a gym membership. However, mother can't afford it due to various financial challenges including repairs needed for their special needs van.  Student doesn't show interest in many activities but initiated this request himself. Mom has also been working on her health and is attempting to make dietary changes for the whole family to support their wellbeing. Student is too young for the free Planet Fitness summer membership.  Mom would have to get a plan and take student with her. The price for the Black Card membership, which would allow mother and student to enter together, would be $75 for both the month of June, including start up fee, monthly fee, and annual fee (all of these are required to initiate a membership) and $25 for the month of July. A gift certificate for Planet Fitness, if they offer them, would be best, or a cash card to reimburse parent if possible. Thank you so much for your consideration.

We have a wonderful family of 5 that has been experiencing financial difficulties for several months now. Dad is the sole provider for the family and has seen a reduction in the number of hours he is able to work. With the school summer break approaching the family is very concerned on not having sufficient funds to provide food for the family. The three children will be at home during the summer break and will need breakfast, lunch, and dinner. We are submitting a request for support in securing food assistance which would lift a huge burden from them and the three students overall wellbeing. A food voucher of $500. to assist the family in providing meals, this assistance would greatly help the family with the nourishment of their daily meals. Client now receives monthly benefits, which she is required to save for potential relocation expenses. Client's daughter is in need of clothing because they arrived at the shelter with only the clothes on their backs. I collaborated with a local community partner to secure mothers' other needs for the food insecurity. Daughter is three years old and would benefit significantly from having appropriate attire for a young child to attend daycare daily. This will alleviate some of Mother's stress as she strives to obtain employment. The mother is a part time college student at a local community college and is working each day to give her daughter a better future.

NMD has been in the foster care system since the age of 2015. NMD was born in another country, brought to the US and abandoned by her adoptive parents a few weeks later. Due to the complexity of NMD’s Mental Health issues, she was placed in a group home. NMD remained in the group home for years, NMD was well liked by staff and helped her eventually transition to a foster home where she currently remains. NMD is a Regional Center client and has blossomed with their help and guidance. NMD will be turning 21 in a few months and will be transitioning out of the foster home into independence. Some of the strengths that NMD has is that she has maintained stable employment in her first and only job. NMD has been at her job for over 1.5 years and has been commended at work for her strong work ethic. NMD also graduated from High School and is fluent in English. NMD has made it a goal to save a large part of her paycheck in preparation towards independence. NMD’s goal is to live on her own, continue working and have a prosperous life in this country. NMD has been able to benefit tremendously from the services provided by Regional Center, she has obtained a full time (graveyard shift) job, is learning to budget her money, learning how to cook and relates well to others. NMD’s Regional Center worker helped her find an affordable 1 bedroom apartment, which NMD will be able to pay on a monthly basis. NMD is in need of a Refrigerator, TV, Blender, Toaster Microwave and Coffee Maker.
