The mother is recently a single mother due to the father being intoxicated while caring for the children and not being able to meet their needs for supervision. She is seeking employment because her seasonal job ended. The father does assist when able, but they continue to struggle to fully meet the needs of their children. The family is in need of grocery items. Milk, Fruit, Breakfast food, Bread, PB and J, tea, Kool aid, snacks for the children, items to cook.

Two children are placed here with relative from Indiana. The family recently lost their car. The children go to school out of district due to School Choice program. The relative has no way to get the children to school without a car. She has a rental car through tomorrow, but can't afford it after that. We are requesting transportation assistance to help these kids get to and from school until the end of the school year. Any assistance would be greatly appreciated. I'm not sure what to put for exact need. They need a vehicle but can't afford one right now. But the main issue is getting the kids to and from school.

Due to medical reasons a FP is asking for some time off. We are moving the child to another NYAP home and the new home needs a trip bunk bed to accommodate this child. The FP already has three children in her home. This will be a 4th placement. She is moving to a larger (3 bedroom) apartment and needs to furnish the room with a bed that can hold the children. A triple bunk will help to maintain this placement and help us to facilitate this new placement.

This single mother has just been reunified with her 2-year-old son. Mom has been working hard to keep a job and a home for herself and her boy. She has been cooperating and making all the needed changes to be reunited with her child. Mom is requesting assistance with acquiring a convertible car seat for her son because he is quickly growing out of the one that she has for him. Mom wants to make sure that his car seat meets legal standards for his size, and to make sure her son fits comfortably in his car seat. This will help the family get to appointments and daycare safely.

This request is for a crib for a 5 month old. Two children were placed in the caregivers home with a non-relative and they came with nothing, no clothing, diapers, Wipes or car seats and only one opened can of formula. The caregivers have a toddler bed for the 2 year old so we are only looking at needing the crib and mattress. The formula needed is Nutramagin-White Can-Hypoallerginic. They are requesting WIC but it will be several days before they can get that started. The 5 month old wears size 2 diapers and the 1 year old wears size 6 diapers. Wipes of any kind would be appreciated. They need clothing with both wearing size 2. Last most important besides the formula is car seats. The 5 month old needs a 5 point harness, rear facing infant car seat. The 2 year old needs a forward facing, 5 point harness car seat. The caregivers stated they welcome hand-me-downs and will pass it forward if whatever they get doesn't fit or in the meantime have gotten what they needed.

The family refrigerator went out a few weeks ago causing a gnat issue. The family is need of a refrigerator.

A father and his 3-month-old have just obtained an apartment of their own. He has done an amazing job with setting up daycare and remaining employed all while taking on the single father role. He has been great at using local resources to obtain a crib, some crib sheets, burp clothes, a baby swing, a walker, a bumbo seat with a tray, bassinet, diapers and wipes to help him get successfully set up in the caregiver roll. He has also been able to use a network of friends and co-workers, as well as local transportation outlets, to help get him to and from work and daycare. We are hoping that the community would be willing to come alongside this father in supporting him by helping to provide the final items he needs which are a washer and a dryer. This would help him out tremendously. The father has been so grateful for all the help that he has received and would be so appreciative if this final need could be met. We have an amazing opportunity to make a lasting impact on this father and child. Would you please consider helping this devoted father in his time of need?

This family needs assistance in obtaining items for their home in order to reunify with their 2 children. The family home needs the following items in order to safely reunify the children: carpet for their children's room, a dresser, a door for their bedroom, two twin size bed sets, cabinet locks for their kitchen, lock box for the children's medication, and an electric stove.

This family has just moved into their own home. Dad is unable to work due to being disabled. Mom works at a local grocery store and is the sole provider for the family, while dad stays home with their two daughters. The family did not have many furnishings or home items of their own. Dad asked for help with a small dining room table and chairs so that the family can eat meals together at the table instead of in the living room. Dad also let the support coordinator know that the family needs dressers for their clothes, as the clothes are piled up around the house at this time. Dad said that the children need a clothes hanging rack for their little clothes. The home appears very cluttered with the clothing everywhere so the dressers and hanging system will help tidy things up.

This young family is still in need of these items to help them thrive in their new home.

This request is for 2 mini cribs and mattresses. This is for a 1 and a 2 year old. The children are placed in a non-relative home with their sibling and the caregivers 3 biological children. This makes it a total of 6 children in the home. The caregivers need to have mini cribs so that they will fit in the bedroom they will be sharing with their biological 1 year old child. They also need a tall baby gate 55.1 takk with adjustable from 29.13 to 42.08 wide, to keep the 2 year old from climbing over the gate and going up the stairs or down the stairs. The family thought they would be helping out the mother for a little while but it appears it is going to be a lot longer so now the family needs more permanent sleeping arrangements.

This family also needs diapers and wipes. Dieapers are size 4 & 5. There are 2 children age 1, and one child age 2. The 2 year old wears size 5 and the 2 one year olds wears size 4. They have not started getting any funds yet and they are really struggling financially.
