Caregiver/NMD needs a refrigerator. Caregiver has been trying to save up for one but has not been successful. Although, caregiver is having that challenge, caregiver is trying to her hardest to provide for her children. Furthermore, caregiver is committed to maintaining the family together. During my work with caregiver I have identified the following strengths: mother works, is knowledgeable of child’s needs and is present.. Caregivers goals are to be able to live on her own. For these reasons, I am advocating for caregiver to have the need identified met. The main reason I am requesting this need to be fulfilled is because mother is having a hard time to work and then stop by the market and cook fresh everyday for the 3 children and herself everyday. Mother and children were kicked out of her old apartment and was not allowed to remove anything from the location. Mother lost all her appliances and everything for her kitchen. Mother stated losing everything was hard but the worse thing is not having a fridge to save food. Food has been spoiling if they don’t eat it right away. Mother does not get CAlfresh and is buying her food by her hardworking money. This need is critical to ensuring that family can stay fed and save money. DCFS has assisted the family by providing a referral for kitchen items to Jamesstore. DCFS will continue to link the family to community services such as food banks, counseling, etc. in order to ensure the family continues in maintenance.

Young mother and her two children are in need of furnishings to accommodate the growing needs of the family and unable to afford it due to the family receiving limited funds. The mother is currently focused on starting a nursing program and at this time is unable to work to afford this large expense.
The family is in need of a queen size bed frame, box spring & mattress (their current bed is very old and too large for their space-it was handed down to them and is showing some wear and uncomfortable to sleep in), and a tall drawer dresser (they are currently renting a room and need storage for the children's belongings).
By fulfilling this request you will help the family have an organized space, and get good rest so they can be comfortable and well rested.

This family consists of a mother and two children are in desperate need of a working refrigerator. The mother has been searching online for a refrigerator, but has not been successful with finding any within her budget. She also does not have a way of transporting a refrigerator, making the prices higher due to delivery charges. Although, she is having that challenge, she is meeting the case plan goals/court orders and recently received custody of her children again. The mother is working full-time and is resourceful, resilient and caring. The main reason I am requesting this need to be fulfilled is because it aids in preserving food, a basic necessity for living. Her current refrigerator broke and only the freezer part is working. The family is currently having to make multiple trips to the grocery store a week. This need is critical to ensuring the caregiver is able to budget and provide adequate and fresh amounts of food for the children. This need is being requested through Care Portal because unfortunately DCFS would be too long as some requests take up to 8 – 10 weeks.

We are asking support of our local community for a mother who has successfully reunified with her two minor children. Additionally, mother has a young adult child who is going to school, and helps the mother with the rent and bills. Mother worked every day to be as involved as she could while her children were placed with relatives. Mother currently does not have a kitchen table for her kids, and since reunifying the bond with her sister has been strengthened, as she was the relative caring for mother's children at the time. The maternal aunt and her children have been visiting the children a lot since the reunification.
Mother works at a cafeteria, and is providing for the children's needs and extra-curricular activities, which is many. Her children are in early adolescence (middle school) and the separation from mother was very hard on them since mother has been the only parent in their lives.
We would love if this family could be blessed with a new table and chairs set, and a large couch so the whole family can comfortable visit, the children can do homework, eat, and spend the end of the year together.

Furthermore, caregiver is committed to her children and maintaining the family together. During my work with caregiver I have identified the following strengths: Mother is resilient, Children are healthy and well cared for, mother is loving and bonded to the children. Caregivers goals are to continue having children assessed for services as needed, maintain regular medical care, and to continue to maintain a healthy and safe living environment for the children. For these reasons, I am advocating for caregiver to have the need identified met. The main reason I am requesting this need to be fulfilled is because the children currently have inappropriate sleeping arrangements. It is important for the overall safety and wellbeing of the children to ensure that they have a safe place and appropriate place to sleep at night. Mother works Tuesday-Friday from 3:00pm-9:00pm across the street from the home due to needing to also be close by due to maternal great aunt being elderly. Mother does not have a working refrigerator and is using a mini one to store all her food, she is unable to purchase anything due to her very limited income. Currently they are unable to store much in the fridge due to it being extremely small. Mother is also struggling with buying groceries and on occasions goes to food banks to obtain food.

Mother hopes to receive beds during the holidays to bring a sense of joy into her children's life's after these difficult times. Mother is working on utilities and additional furniture for their house. Receiving four twin beds and bunk bed would help mother with additional support in stability. The help will provide another level of comfort and support for the mother and family. Receiving beds will assist family to have a safe and warm bed during these cold nights and will help the house feel like a home to them. The client does not drive, and the family only has one vehicle that the partner uses to travel to work. The client is in need of a Stoller for the baby, who has been rapidly growing, and would benefit the baby and client getting to and from places they need to go. The client has been reaching out to resources and links given to her, and after many attempts have been made, no Stoller has been obtained. The client has been with the program for a little over five months and has complied with the program. The client is open to home visits and has completed some of her goals. The client has always reached out to resources given to her and has been able to obtain other baby items. The client is thankful for the services and support received. If the client receives a stroller, the baby will be able to continue her milestones and be a healthy baby along with her mom. Thank you.

We have an amazing family that has been facing many hardships the last couple of months culminating in the eviction moratorium ending. Single mother of two amazing children is currently struggling to find Section 8 housing close to her children's schools and has no transportation/ car to facilitate picking up and dropping of kids and seeing apartments.
The students are amazing students at our Elementary schools and have been suffering from the lack of future housing stability. We are currently working with mom and community organizations to assist in finding adequate housing.
We are requesting assistance in paying back rent as well as a grocery card voucher for her to be able to continue providing healthy food for the family while attempting to save for the down payment. These two sponsorships would provide a huge opportunity to mom in being able to keep the wellbeing of the family at the forefront while allowing Mom to focus on the down payment assistance.
