3 y/o male child placed in home with fictive kin and older sibling. He is in need of clothing size 3-4 T, a dresser, pull ups size 5, wipes, socks and shoes size 10. Family may identify other needs once contacted by care portal team. Any help for this family would be greatly appreciated.

Spanish speaking 3 y/o female child placed in care givers home with no supplies. The family is not known to the caregivers, but they are pastors, and they offer support to our Spanish community that do not have any other family or resources. They offer support to the parents as well as to the children in need. They are in need of the following items for the child:
Pull-ups size 6-7 (child wears at night only) Food(s)- child eats fruits (all), whole milk, and ground beef Clothing-sizes 4-5T Dresses, underwear to encourage potty training, shoes size 9 Toddler Car seat for a 6 y/o child that is also in the home at this time Any help provided would be greatly appreciated in order to keep this placement stable. The caregivers are able to communicate with the child and are here to support the family when appropriate to do so.

Eight-year-old male child placed with relatives in need of clothing. Child wears Mens Medium shirts, pants, and 7 1/2-8 in mens shoes. Came to care givers home with very little any help would be greatly appreciated.

Mom and Dad are trying to make ends meet while raising 5 children between the two of them. Mom has been trying to find a job but has also had vehicle trouble, so transportation is a hurdle for them. Their power was just restored and the family needs groceries. Mom says the family could use some meats, fruits for snacks for the kids, bottled water, canned items such as raviolis and spaghetti-o's and frozen pizza. One child was present during conversation and told Support Coordinator that he likes soft tacos, apples & macaroni and cheese. The family could greatly benefit from not only the groceries but a connection to a local church body.

Single parent is currently on unsupervised visitation with 3 children. Parent is moving forward and going to be starting overnights into reunification as a single parent reunification. The parent has purchased 3 twin bed frames but is needing mattresses. These mattresses will provide for the children appropriate sleeping arrangements allowing for the parent to begin overnight visitation and be reunified.

The family refrigerator went out a few weeks ago which is causing a gnat issue.

The mother is a single mother who works from home while residing with maternal aunt and other children in the home.

The relatives are not part of the case. The mother is doing the best she can. The refrigerator going out was an

unexpected cost for the mother, and she does not have the money as she is the sole provider for the children. She

needs the refrigerator to ensure the children have adequate food in the home at all times.

Two children are placed here with relative from Indiana. The family recently lost their car. The children go to school out of district due to School Choice program. The relative has no way to get the children to school without a car. She has a rental car through tomorrow, but can't afford it after that. We are requesting transportation assistance to help these kids get to and from school until the end of the school year. Any assistance would be greatly appreciated. I'm not sure what to put for exact need. They need a vehicle but can't afford one right now. But the main issue is getting the kids to and from school.

Due to medical reasons a FP is asking for some time off. We are moving the child to another NYAP home and the new home needs a trip bunk bed to accommodate this child. The FP already has three children in her home. This will be a 4th placement. She is moving to a larger (3 bedroom) apartment and needs to furnish the room with a bed that can hold the children. One set of twin bunks are needed

The Foster Parent has stated that regular bunk will work. We are requesting one set of bunk bed frames and the mattresses.

This single mother has just been reunified with her 2-year-old son. Mom has been working hard to keep a job and a home for herself and her boy. She has been cooperating and making all the needed changes to be reunited with her child. Mom is requesting assistance with acquiring a convertible car seat for her son because he is quickly growing out of the one that she has for him. Mom wants to make sure that his car seat meets legal standards for his size, and to make sure her son fits comfortably in his car seat. This will help the family get to appointments and daycare safely.

New or gently used items are accepted and always appreciated.

This family has just moved into their own home. Dad is unable to work due to being disabled. Mom works at a local grocery store and is the sole provider for the family, while dad stays home with their two daughters. The family did not have many furnishings or home items of their own. Dad asked for help with a small dining room table and chairs so that the family can eat meals together at the table instead of in the living room. Dad also let the support coordinator know that the family needs dressers for their clothes, as the clothes are piled up around the house at this time. Dad said that the children need a clothes hanging rack for their little clothes. The home appears very cluttered with the clothing everywhere so the dressers and hanging system will help tidy things up.
