The mother just relocated from North Carolina to Phoenix, Arizona, with her sister. She was a victim of a domestic violence dispute. She was able to get away, obtained an order of protection, and relocated to Arizona to ensure her and her child's safety. Since she had to travel across state lines, she has very minimal baby supplies and is in need of more. She will also be referred by the Department to other community services since she is new to Arizona. The mother traveled and was not able to bring only limited items. She needs a bassinet, baby bottles, diapers size 4, wipes, she would like a car seat and clothing is a size 12 months.

This single mom of 3 is requesting clothes for her younger child who is need of summer clothing and shoes. She has been struggling to find a job and her child need clothing. The child is a male and is in need of short sleeve shirts size 10/12, stretchy athletic shorts and pants size 10/12, and sandals size 2 in boys.
Mom requesting no jean material due to them being too tight on the child and uncomfy.

The mother is a single mother. She was recently in a domestic violence relationship. She was able to obtain an order of protection and leave to reside with her mother. She is not employed and is currently pregnant. She is in need of baby items just to help her get back on her feet. She would need a car seat, stroller, boy baby clothing, 0-3 months, baby bath items, baby shampoo and a baby carrier. The mother expressed she has been unable to get any items because she has separated from he father. She is able to live with her mother until she is able to be on her own. She is also asking for her for diapers and wipes.

The mother is a single mother trying to make ends meet. She is currently living off two months worth of savings to meet her basic needs. The father was recently detained by ICE and may be deported. The father was the financial caretaker. The mother is the primary caretaker for her youngest son, who was born premature. The mother has to care for the child 24/7 as he is on oxygen and several different medications. The child is one year old, but the doctor measures him at 8 months. The mother is requesting a little help with gathering clothing, diapers, and wipes for her children.

Her daughters wear a size 12 in women's. Both daughters wear the same size. One daughter wears a size small in women's shirts and the other daughter wears a size medium in women's shirts The baby wears size 4 in diapers and would need some wipes.

The mother expressed not wanting to be a burden but does need some assistance. The mother knows the children will need new clothing for the up-coming school year and just in general. She is limited resources and unable to work due to her youngest with serve medical needs

The mother is in need of help getting her children some clothes for the summer. She is a single mom and had to cut back her hours at work to take care of her children's needs and can't afford to get them new clothes on her own. little girl size 8T or small Teen girl size M in shirts and 9 in pants Teen Boy size 34-26 pants and M in t-shirts

Grandmother took in her two granddaughters - ages 11 and 13. Only the 11 year-old resides at home with grandma. There are a couple doors, a window, and holes in the wall that need repairing. Grandma is in a wheelchair and only works part-time. She takes care of her granddaughter's needs and is very strong in her faith. This would greatly help grandma, as her income is limited and there are other bills she currently has to pay.

A single mother of 2 (ages 1 and 5) is needing assistance in getting a safe, reliable 5-point harness for her 5 year old who has autism. The 5 year old is a very vulnerable child, and does not understand simple safety instructions, such as to not get out of the car. She needs a more secure car seat as she currently only has a booster seat. She has now learned how to get herself out of her booster seat and has been placing herself in dangerous situations, however she is unable to recognize the danger. By helping this mother get this car seat, you are helping to ensure this child gets to every place she needs to be safely. Mother will be able to reuse this item for her 1 year old as well.

This family is also in need of a high chair for the 1 year old. Mother does not have any dining items, such as a table/chairs. While she has made do without that, when she has to feed both children it can be very difficult. Her 5 year old is a very active child. If she has a high chair, she will be able to have a safe place to sit her 1 year old during feeding times while also managing her 5 year old. She is doing this alone, and any help she could get would greatly strengthen this family.

The household is Mother, her 7 children, and 1 child that was placed with her. She is fictive kin for the child. The Mother is looking to get a twin size bed for the child and some bedding for him. He is 13 years old. There is no preference on anything.

The department observed the home, and found that there was minimal furniture. The mother listed: blankets, couch or pull out couch, baby high chair, coffee table, 2 full size beds, diapers for size 3 and 4, dishes, hygiene, household items, lamps, dresser, hangers, curtains, and pillows, as things that she would like assistance in getting. Having these items would assist in her ability to maintain a healthy and structured environment for herself and her children.

The child is 17 year old teenage who is being adopted by his 20 year old brother. His older brother works really hard, often picking up extra shifts, to provide for the teenager. The teenager is a sophomore in high school and hopes to start working also. The teenager is in need of a twin bed and bedding.

Father is a single-parent of five young children including a newborn child. Mother is currently not involved as she currently struggling with substance use and homelessness. Father is in great need of three queen size beds for his children to sleep in. Father has been able to obtain queen bed frames from the community and social supports, but has not been able to locate the correct size bed. They are currently residing in a two bedroom home. The children are currently sleeping an air mattress to sleep throughout the night. Father will be extremely grateful for any assistance for this matter. The children will also be grateful to be able to sleep on a solid bed for the night.

A single mother is needing a box frame or something similar for her child. She was able to get a queen mattress for him and is needing the bottom portion for him. This mother has finally obtained housing after being denied numerous times. This mother has not given up to ensure she reunifies with her child.
