Mother needs a co-sleeping bassinette. Mother has been co-sleeping in a twin bed because mother resides with extended family members and the home does not have space for a bassinette in the room that mother shares with relatives. Mother has been co-sleeping with the newborn as a result of this issue. Mother is currently in an open case for her other children and has been working hard at meting her case goals while juggling a difficult pregnancy and now caring for a newborn. Furthermore, caregiver is committed to reunifying with her older children and maintaining custody of the newborn. The main reason I am requesting this need to be fulfilled is because it will help mother provide a safe environment for the newborn and meet the request of DCFS to ensure that mother is not co-sleeping due to the risk it poses to the newborn. Without the co-sleeping bassinet mother is placing the child at risk and DCFS will have to intervene or mother will have to leave the newborn in the bedroom of her extended relatives and not be able to properly monitor the newborn overnight. This need is critical to ensuring that mother is empowered to properly care for the child and build crucial parenting skills that is obtained during this delicate stage. This need is being requested through Care Portal because unfortunately DCFS does not provide this item. The estimated cost of the need/s being requested is $140 to $230 depending on the product or brand purchased.

The client is a single mother of two girls who recently lost her husband. She was forced to move out of her home and into her vehicle due to severe mold in the residence, which the landlord has refused to address. She was advised not to take any belongings with her, as the mold had spread to her clothing and was considered hazardous. The client has been receiving services from SHIELDS for the past seven months and has been attempting to access a housing shelter. Despite her circumstances, the client consistently attends doctor appointments for herself and her children, as they have been affected by mold exposure. She is currently unable to work but is seeking clothing and blankets for her children due to the extremely cold nights. Help Support a Youth Aging Out
We have a 19 year old Non-minor Dependent who was admitted to the Emergency Room due to having loss of sensation in her leg. It was discovered that she has a tumor on her brain. The NMD will be released soon from the hospital, and she can only lay at a minimum 30 degree angle and she will need an adjustable bed upon her release from the hospital. This NMD is in need of immediate support in securing the adjustable bed, mattress and sleep bundle. The NMD is a really good kid, who is working to obtain her bachelor’s degree. Prior to this incident, she was working but ultimately had to quit her job recently so that she could focus on her recovery. Please help us help this Non-minor Dependent with her recovery by providing this special adjustable bed and mattress and sleep bundle.

Client tried to work out a payment plan with DWP effective April, 2025 but she does not receive enough income to cover rent and increased utility bills.
