Father has been very consistent with the Court Ordered case plan. The father’s progress has demonstrated he is capable of changing his lifestyle. The court recently ordered overnight visits with the children. The Department is currently assessing father’s visits so that we can liberalize and release the children to father. Father is employed through a temp agency and has been consistent with his employer for about three months. Father currently resides in an outpatient housing program with another program roommate so the children would not be able to live there. Since the father is working a full-time job, he is trying to save money to find his own housing so that he can have a home for the children. Father has a suspended license due to unpaid DUI fees and DMV is asking him to pay $1400 and take classes to reinstate his license. He has a vehicle but is not able to drive it. This is challenging for him when trying to pick up kids since he has to find someone (pays them) to drive for him since he cannot drive. The children are very bonded with father and are very happy when they’re with him. He is very hands-on with the children. The social worker has talked to his program case manager and staff and everyone has nothing but positive things to say about this father and his successful progress. He is demonstrating that he is motivated to make a change in changing his lifestyle and any support would be helpful to assist him with reunifying with his children successfully.

Mother is a single mother due to father getting deported about 5 years ago. Mother is unable to receive any government assistance due to both children being undocumented. She has little to no family support and only has an maternal great aunt who helps her in the afternoons with caring for youngest child. Mother works Tuesday-Friday from 3:00pm-9:00pm across the street from the home due to needing to also be close by due to maternal great aunt being elderly. Mother does not have a working refrigerator and is using a mini one to store all her food, she is unable to purchase anything due to her very limited income. Currently they are unable to store much in the fridge due to it being extremely small. Mother is also struggling with buying groceries and on occasions goes to food banks to obtain food.

We have a 5th grader and parent in need of support, the single parent is the sole provider and working in retail with not many hours of work available to her. They rent a room but will need to move out very soon. The owners of the house where she rents a room have divorced and now per court orders must sell the house. This parent could use support with a money card to provide basic needs for the 5th grader. Not knowing where they will go next she wants to provide a jacket and shoes along with other clothes to keep this child warm. She also could use financial support to provide healthy food to keep child and mother well. A gift card for Walmart would give them assistance with clothing and food at one location. Mother has been receiving basic needs services at local Family Resource Center has recently moved from a DV Shelter to a 2 bedroom with her 2 . Mother was given a bed that the family has been sharing. Family has 1 complete bed they sleep on together, recently mom was given 2 twin mattresses and bed frames but no box springs. The bed frames require a box spring to hold up the mattresses. Family would benefit from two twin box springs and dressers for clothes & belongings. Parent is in need of four twin beds and a twin bunk bed. Due to current status, children sleep on an inflatable mattress on the floor. Mother hopes to receive beds during the holidays to bring a sense of joy into her children's life's after these difficult times. Mother is working on utilities and additional furniture for their house. Receiving four twin beds and bunk bed would help mother with additional support in stability. The help will provide another level of comfort and support for the mother and family. Receiving beds will assist family to have a safe and warm bed during these cold nights and will help the house feel like a home to them. Working with a parent who is part of the DCFS workload, the worker reached out for a bit of assistance with his mother's situation. We helped create a plan that ensured the mother/minor's safety and well-being. The family is currently homeless and living in their car. Mother has stage two cancer and custody of her children. DCFS is currently in the process of securing stable housing for the mother. However, it will be two more weeks before the family can move into their home. As a result, the worker reached out for assistance in any way she needed, such as paying for a hotel room. DCFS is able to provided coverage for a week. And we are wondering if someone might be able to cover the second week which would be 11/28/2023 through 12/05/2023. The cost is $99 per night, a total cost of $693 for 7 nights. She is currently at the Bellflower.

A three year old boy is approaching overnights with the father to reunify after the agency became involved due to allegations of neglect by the mother and father. The father has been applying to new positions as he currently has two interviews scheduled. The father has been doing small jobs in the meantime so as to continue to have some type of income. The father has had the support of his family whom have provided food and other utilities for him. The father has also reached out to local churches and or free produce (fliers) the undersigned has provided him with. The father is in need of $500 for his November rent. If granted the funds to help with one month’s rent, the father will be able to catch up with rent and continue to make his payments afterwards, thus enabling the father to continue to progress, be able to provide stability for the child, and continue with liberalizing the father’s visits to overnights by the end of the month and eventually reunify.

This family has been participating in the SHIELDS Family Preservation program since September 2023. The mother of two small children are requesting assistance with the purchase of a highchair, a walker, and a large toilet training seat. The 10-month-old infant was born prematurely, so he is developmentally delayed. A highchair would benefit him in being able to sit and learn how to feed himself. Additionally, a walker would benefit the infant in being able to strengthen his leg muscles, as he prepares to learn how to walk. Lastly, a large toilet training seat is being requested for the 2-year-old toddler, as she is overweight and unable to fit in a standard toilet training seat.

Hello, I am making this request for a family of 9. This family recently reunited and the Department is working on stabilizing the family. The mother was provided housing by the Family Reunification Program, and is in great need with assistance to furnish the home. Previously, the mother and the children were living with the maternal grandmother, which was causing a lot of discomfort, due to there being a large amount of people living in the home. The children have stated they are thrilled to be living in a home for themselves, and are looking forward to making their new home, feel like home. The mother is currently employed, but is only making enough to support the children and additional bills. The children's ages include: 15, 14, 13, 11, 10, 9, 8, 5. The mother is hopeful she can have a large sectional couch, in order to be able to sit the children during family time.
A child that is five has been reunified with his father. He and the father sleep in the same room. The father is currently rebuilding his life as he now sleeps in a twin bed. The family is requesting a toddler bed and mattress for the youth so he and the father can sleep comfortably.
The client is a single mother who recently ended her relationship with the father due to ongoing domestic violence. The mother is strong a resilient. To continue to ensure her child's safety, the mother would like to purchase cameras to place in her window to provide further surveillance for her home until she can move to a new apartment, however, she currently does not have the funds for the expense. As such, I am requesting Target/Walmart/Amazon gift cards to assist the mother with the cost of cameras.

This is a CRITICAL situation. I have a family who is being evicted from their home today, November 21, 2023. The mother reported receiving an eviction notice. The mother informed the landlord of mold in the home and refused to pay the rent because the landlord has not repaired the mold in the home. The mother and her two children are currently moving out of the home. The family has no available supports to provide shelter. I have referred the family to Bringing Families Home but this may take some time. Wraparound team is unable to provide Emergency shelter because they also need time to put in the referral. Assisting with emergency shelter until she can receive assistance is crucial to the children’s safety and wellbeing to ensure they have a roof over their heads and a warm place to sleep, especially considering the drop in temperature. One of the children has mental health issues, which can be exacerbated with this current instability. Any help is greatly appreciated. Please advise.

Good afternoon, I am the Case Manager of a student who is in current need of a new bed frame, and a mattress. The mother's finances are currently restricted, and the child reached out to me, in an attempt to alleviate the mothers' financial strain. The child we service is receiving mental health services. The child has excelled academically over the course of the academic year, but the disturbance in her sleep is causing a decrease in her academic standing. The child cannot make use of her current bed any longer, hence it is broken. The family's financial restrain has made it difficult to purchase a new bedframe, and a mattress for the child. I have spoken to the family and determined that the child will benefit from having a place other than her broken bed obtain adequate rest. Due to the clients size, she needs a bigger bed, in her case a queen bed. The child and her mother are consistent with reaching out for support and our organization is devoted to helping her and her family reach their overall gals.
