The mother just relocated from North Carolina to Phoenix, Arizona, with her sister. She was a victim of a domestic violence dispute. She was able to get away, obtained an order of protection, and relocated to Arizona to ensure her and her child's safety. Since she had to travel across state lines, she has very minimal baby supplies and is in need of more. She will also be referred by the Department to other community services since she is new to Arizona. The mother traveled and was not able to bring only limited items. She needs a bassinet, baby bottles, diapers size 4, wipes, she would like a car seat and clothing is a size 12 months.

The mother is a single mother trying to make ends meet. She is currently living off two months worth of savings to meet her basic needs. The father was recently detained by ICE and may be deported. The father was the financial caretaker. The mother is the primary caretaker for her youngest son, who was born premature. The mother has to care for the child 24/7 as he is on oxygen and several different medications. The child is one year old, but the doctor measures him at 8 months. The mother is requesting a little help with gathering clothing, diapers, and wipes for her children.

Her daughters wear a size 12 in women's. Both daughters wear the same size. One daughter wears a size small in women's shirts and the other daughter wears a size medium in women's shirts The baby wears size 4 in diapers and would need some wipes.

The mother expressed not wanting to be a burden but does need some assistance. The mother knows the children will need new clothing for the up-coming school year and just in general. She is limited resources and unable to work due to her youngest with serve medical needs

The department observed the home, and found that there was minimal furniture. The mother listed: blankets, couch or pull out couch, baby high chair, coffee table, 2 full size beds, diapers for size 3 and 4, dishes, hygiene, household items, lamps, dresser, hangers, curtains, and pillows, as things that she would like assistance in getting. Having these items would assist in her ability to maintain a healthy and structured environment for herself and her children.

The child is 17 year old teenage who is being adopted by his 20 year old brother. His older brother works really hard, often picking up extra shifts, to provide for the teenager. The teenager is a sophomore in high school and hopes to start working also. The teenager is in need of a twin bed and bedding.

A single mother is needing a box frame or something similar for her child. She was able to get a queen mattress for him and is needing the bottom portion for him. This mother has finally obtained housing after being denied numerous times. This mother has not given up to ensure she reunifies with her child.

A single father is in need of help obtaining a bunk bed for his daughter. A bunk bed is needed that has a full bed on the bottom and twin on the top. This father has stepped up and hit the ground running after being released from his incarceration.

I am working with a single mother to five children. She is very resourceful and doing everything she can to provide for her children. Her children are very close with their mother and each other. The family could use some assistance with things like hygiene items for the children, or even cleaning supplies.

Currently, the family is just moving into their new apartment. The family was needing housing to finally start the reunification process with their children. The family also has a new baby that was not brought into care. Currently, 3 of the children are placed in foster care. The family would be eligible for overnight visits in the near future if they are able to get the furniture needed. Please reach out with any questions.

This request is for a mother who recently had a newborn baby. She has had some financial struggles lately and doesn't have a mode of transportation. It would benefit her to have a stroller and baby carrier so that she can safely take her child out into the community.

A single mother of two teenagers recently moved to Phoenix from Oklahoma city. She moved to help provide for her family and get access to more services for her daughter with behavioral health needs. The mom works full time to meet her child's needs but has been unable to afford needed furniture for home and her children. Her daughter struggles with mental health needs and behavioral outbursts. The mom has been had her enrolled with multiple behavioral health services and is focused on her needs when she is not working. The mom has a strong connection with both children and tries to prioritize spending time with them and having fun wit them when she is off from work. This family is in need of a couch and a dinning table as well as a bed for herself. The home they are staying in is currently unfurnished with only a bed for each child.

This family was recently separated by DCS, and this mother has been working hard in order to reunite with her children. She has been able to secure an apartment but is struggling financially and needs help furnishing it. She wants to provide a safe and comfortable home for her children to return to. This would also include clothes and shoes for her children. She currently has 4 children, including an adult daughter who is expecting, two sons and another daughter. Their sizes for clothing and shoes are listed below.

• The bunk bed that is listed should be a twin on top and queen on bottom, in order to fit the family's need.

The child is a 15-year-old teenager. She is placed with her second cousin, his wife, and their two children after spending several years in a group home. She struggles with her weight and has successfully lost a considerable amount of weight. Her caregivers could use some help getting her some new clothing. Things she needs :
