This is a single-mother family who has a child with multiple medically complex diagnosis. The child is 13 years old and diagnosed with cerebral, seizures, microephaly, dysphagia, and a feeding disorder. This child is non-verbal and does not qualify for Indiana Medicaid. The child has never attended school due to not having a wheelchair or any way to be mobile while at the school. The family has just been approved and fitted for a carseat through NuMotions, but needs an additional $150.80 to help cover the cost for a specialized part. This payment would help secure this carseat for this child, and open up the option of attending school and getting the child more accessibility.
DCS is working with a family that is comprised of a single Mom and 2 children . The children are half siblings. One child is 16 and is in high school. The other child is in elementary school. This Mom reported herself to the department as she was having trouble parenting the 16 year old. He was in trouble with the criminal justice system and would not comply with requests from Mom. The youth would also destroy items in the home. The youth spent a stint in the Madison County Juvenile Detention Center. The youth was released April 6, 2023. The family has done surprisingly well. The mother and youth have made some progress in the services for which they have been referred. Mom has been welcoming and compliant with the FCM and all requests from the department. They are currently living next to neighbors who have repeatedly called in reports to DCS this FCM knows to be false. The family antagonized Mom. Mom has handled things well as she reaches out to FCM, the Housing Authority and law enforcement. Service providers have helped Mom secure a home to rent. Utilities cannot be transferred until a past due amount of $160 is paid. This new housing opportunity would give the family a fresh start and allow the youth to have weight lifting equipment and a punching bag in the garage. FCM believes anything that can be done to help facilitate this family's move to the new location would strengthen the family and have a positive impact on the case and the family.
TFL is a single female relative caregiver with four children in her home, two of which are involved with DCS. TFL holds three jobs while doing her best to make ends meet and to also give the four girls a memorable and normal childhood. TFL has recently paid for three of the four girls to go on a trip since they had never been on one before and try to plan something they remember. In doing so, TFL has gotten behind on rent and has asked for assistance just this one time. She has plans to make October rent but if TFL does not have September rent made by 9/30/2023 at 4pm she runs the risk of formally being evicted by her landlord. TFL has never been behind on rent in the past and does not plan to be behind in the future. She has not asked for any financial assistance from DCS prior to this request. TFL was embarrassed to ask for help but was the last resort to reach out to her FCS. TFL has attempted to obtain help from the trustee's office, although was told it is a long process and she would not be able to receive the help by 9/30/2023.

The children are five and two years of age. The children were placed back home with mother last week. Mother is employed at a fast-food restaurant but is only making minimum wage. Mother needs assistance with purchasing clothing items for the children now that the weather is getting colder. Any assistance is greatly appreciated.
The children were recently placed on TTV with father. Mother passed away last year. Father in in need of assistance with getting dressers for both children. Father is currently unable to work due to health concerns.
Mother needs assistance with someone putting a tie rod on her car. mom already bought the parts. Mom lost her job due to her car being broke down, and she is the sole supporter for her family.
A single female caregiver is looking for help in obtaining a twin bedding set to take additional placement of a difficult to place teen. With teenage children being difficult to place, we found a caregiver who was happy to take placement but needed a bed for the child. With getting the bed for the child, it will help keep and maintain placement of this child so they are not bounced around or potentially placed in ESC. This need is looking to be met within a normal timeframe - no rush/priority.
9 year old boy has been struggling with bed wetting and needs a new mattress. The bed wetting has been resolved, but the mattress was not able to be saved. Parents are unable to purchase a new mattress at this time.
I am working with a Mother who is working very hard to reunify with her Child (boy, 16 years old). This Mother left a domestic violence situation with the Child's step-father and is starting over on her own. She is in need of some furniture to help make her home more home-like. She is in need of a dining table and dressers.
I am working with two children (boy age 4 and girl age 3) who reside at home with their biological mother. Due to a domestic incident, the children and their mother left their previous residence where they were living with Mother's ex-boyfriend, who was the perpetrator of domestic violence. They are starting over. Mother is 7 months pregnant with a new born due to in November 2023. They are in need of some furniture due to starting over, including a dining table and chairs, couch, living room table, bedroom dressers, and a crib.
This family consists of a separated father of three children. Two children are biological children and one child is connected to him by marriage. There two youngest children's ages range from 4-9 years old. DCS is currently involved to make sure the children's needs are being met. This family could benefit from groceries because the father is struggling with maintaining his hours at work due to lack of childcare. These children and the father do not have any allergies.
Bio-Mother needs a double stroller for her community visits with 3 children 2 of them are under the age of 3. Mother is doing well in her services and remaining substance free. She is improving in her parenting skills and a stroller would help keep children safe in public.
Father does not have car seats for three of his five children. The two oldest children do not need car seats. Father needs the car seats to ensure the children are safe when he transports them. Father needs a car seat with a five point harness for his 18 month old child. Father also needs two booster seats (4 year old and 5 year old).
Father and his five children need some resources to ensure their well-being. Father recently became their primary caregiver and does not have everything they need. All five of the children need shoes and clothes for school. The youngest child only has one sippy cup. The sizes for clothes (shirts and pants) needed include 4/5 kids (girls), 4/5 kids (boys), 18M (boys), adult 10/12 (mens), adult 30/30 (mens). The clothes are all for children but the two oldest children wear adult-sized clothing. For shoes, one child needs size 10 (mens) and one child needs size 6 (mens). Father does not know the shoe sizes for the three youngest children but they are still needed. Father is working on figuring out their shoe sizes.
The children were living with their father as he has custody of the children. Father dropped the children off at mothers' home on 8/28/2023 with only a few outfits for each child. Mother is currently staying with a friend that is helping her get on her feet.

A single mother of a 1 mo. old and 3 yr. old does not have access to transportation. Due to mother not having transportation, she is struggling to make it to doctor appointments, meetings etc. Mother has tried to utilize the Hamilton Country Express but with two little ones who need car seats it is hard for her. Assistance with transportation would ensure that Mother is able to make all of the doctor appointments and meetings she has. Mother has five children ages 4 months, 16 months, 3 years, 7 years and 8 years. Mother is the only caregiver/parent present and has been out of work since giving birth to her youngest child earlier in the year and just move into a new home with the help of her family. Mother is in need of many items to make the household run more smoothly and keep the household up to cleanliness standards to prevent removal. Mother is unable to work right now due to injuries from her labor in April and needs support getting necessary items for the home. A baby bath is needed for the youngest baby. Cleaning supplies is also needed and is under cleaning supplies, and cleaning rags, cleaning spray, disinfectant wipes, lysol and fabuloso.
The family is currently in need of several kitchen appliances including a microwave which would really benefit the family so that they can make quick snacks and meals for themselves while they are working towards reunification with their two children. DCS is involved due to domestic violence and the young family in their early 20s seem eager to begin services such as therapy and domestic violence classes.
A grandmother has recently had both her granddaughters come into her home, and is in need of assistance with clothing and diapers for both girls.

If the grandmother would like to schedule a clothing appointment, she can come in to select the clothing for her granddaughters (up to 20 pieces each; socks and shoes are not counted in the 20).
We are at Operation Love until 3:30 p.m. Thursday, 9/14/23, then available for clothing selection again next week on Monday afternoon 1-3:30 p.m. or Thursday 9-11:30a.m. and 1-3:30 p.m. She can call(###) ###-#### to make the appointment.

4 children were removed from their parents. 2 of the children ages 1 and 3 months are currently placed with their great grandmother and need appropriate car seats. This is an immediate need to ensure grandmother is able to safely transport the children medical and well-child appointments.
