The client is a 35-year-old mother living in a shared apartment with her partner and their one-month-old baby. The client reports persistent emotional distress and low mood, often feeling overwhelmed by her circumstances. Breastfeeding has been challenging, as her baby is sensitive to milk. The lack of access to transportation increases her isolation and limits her ability to connect with the community. Through case management and access to tangible resources, the client can regain stability, autonomy, and social connection. Support in the form of baby care items; including Similac Alimentum Hypoallergenic Formula (Purple Can), 6-month-sized clothing, baby towels, baby wipes, 6-oz baby bottles, an electric breast pump, and a crib cushion—would directly address the baby’s care needs. Stroller and baby carrier would support the client’s mobility and reduce her isolation by enabling outdoor access and community engagement. To promote the mother's emotional well-being, self-care items such as a relaxation candle, facial mask, and lavender shower soap are recommended. ****NOTE: “This request is being submitted by the Center for Family Services of Palm Beach County on behalf of the client.
