The child has significant developmental delays. He is in speech therapy and the parents are in the process of trying to get him a diagnosis for Autism as that's what they believe his delays are. However, they have been on numerous waiting lists to see a developmental doctor. The speech therapist is recommending the child get a tablet that uses the speech touch chat app for at home. He uses one at therapy and is making progress. The parents don't have the funds to purchase a tablet, and the insurance won't pay for it without an official diagnosis of Autism. A relative has stepped in and taken home a newborn baby boy unexpectedly. This little boy experiences tremors from withdrawals. The hospital stated that a swing will help calm him down during these episodes. He has been placed with an unlicensed relative who does not have funds to purchase a swing for the child as they have put their financial efforts into other needs in order to take him into their home.

The Department just returned a 12 year old back home to her mother after an extended placement in Kinship care. Biomother works and would love for the youth to be able to go to a summer camp to be able to make some friends as she doesn't have any in the area. Biomother can not financially cover the camp. Non-Minor Dependent splits her rent with her brother, however her brother has not been able to work this past month due to a leg injury. Therefore, the Non-Minor Dependent needs to pay the whole amount for rent. Although, Non Minor Dependent is having that challenge, non minor dependent is meeting the case plan goals/court orders and is currently working as a CNA. Furthermore, Non Minor Dependent is committed to learning how to live independently and prepare for the future. During my work with Non Minor Dependent , I have identified the following strengths: hardworking, caring, and loyal. Non minor dependent goals are to keep saving and apply to a LVN program. The main reason I am requesting this need to be fulfilled is for Non-Minor Dependent to maintain her housing and avoid any legal or financial penalties. This need is critical to ensuring that Non-Minor Dependent has her basic needs met of having a safe home. DCFS has assisted the family by providing financial assistance with transportation to work, a monthly stipend, and work clothing allowance. DCFS will continue to link the family to community services such as low-income utility assistance, money management, etc. in order to ensure Non- Minor Dependent successfully prepares for adulthood. This family needs a new car seat for their youngest son, who is a little over a year old. Their son is bigger than average, therefore he has grown out of the seat he has now and has his legs hanging off. The family understands this is a safety concern but do not have the finances to buy their son a new seat. I am submitting this request on behalf of a 17 year old high school male that is currently staying at a friends house due to being evicted out of his former home. This student is the oldest of 4 siblings. Mom is staying in temporary housing with his younger siblings and is sending what little finances she has where the student is staying. The student would greatly benefit from attending summer camp and would help to relieve some of the financial burden on mom. Student's father is not actively involved in his life and attending summer camp will give the student access to positive male role models as he continues to navigate his life.
