This request is on behalf of a 17 year old male high school student. December 2024 the student was hospitalized for mental health issues. Student was released into his mother's custody 4 days after being admitted to the hospital. Once at home student was met with mixed reactions about the experience from various family members and was told that he was weak and needed to toughen up. Despite the lack of moral support the student has began to make progress in the right direction with the help of weekly therapy sessions and mentoring from the youth pastor at Crenshaw Christian Center. Attending summer camp would give this student a much needed break from his home life and an opportunity to find much needed support and encouragement among his peers. This young man has attended various college tours and wants to pursue occupational therapy upon high school graduation.

Mother is in need of black dressers, a dining table, and window locks for her children’s upstairs bedroom. Although she desires to purchase these items, she has been unable to do so. Despite this challenge, mother is compliant with court orders and case plan goals by meeting monthly with CSW and ensuring the children are available. A referral was submitted for Family Preservation to assist with time management and parenting support. Mother is committed to maintaining her children in her care and has demonstrated strengths, including consistency with family time, compliance with DCFS, and advocacy for her children. She provides stable housing and is focused on keeping her children safe.

These items are essential to create a safe, clean home environment. The children currently have nowhere to store their clothes, no dining table to eat at, and the upstairs windows are unsafe. Mother recently moved into the home and is working and attending school, but is struggling financially to purchase these items. DCFS is requesting the following: black dressers, window locks, and a dining table. Without them, the children’s safety and needs cannot be fully met. This request is being made through Care Portal as DCFS cannot fund it in a timely manner. Sofa bed and twin beds are requested

The child has significant developmental delays. He is in speech therapy and the parents are in the process of trying to get him a diagnosis for Autism as that's what they believe his delays are. However, they have been on numerous waiting lists to see a developmental doctor. The speech therapist is recommending the child get a tablet that uses the speech touch chat app for at home. He uses one at therapy and is making progress. The parents don't have the funds to purchase a tablet, and the insurance won't pay for it without an official diagnosis of Autism. The app can only be used on apple products. parents are asking for assistance in purchasing an apple tablet. There is an Apple iPad Air 2 Silver (Used) for $107.51 (price before taxes) at Walmart. A relative has stepped in and taken home a newborn baby boy unexpectedly. This little boy experiences tremors from withdrawals. The hospital stated that a swing will help calm him down during these episodes. He has been placed with an unlicensed relative who does not have funds to purchase a swing for the child as they have put their financial efforts into other needs in order to take him into their home.

I am submitting this request on behalf of a 17 year old high school male that is currently staying at a friends house due to being evicted out of his former home. This student is the oldest of 4 siblings. Mom is staying in temporary housing with his younger siblings and is sending what little finances she has where the student is staying. The student would greatly benefit from attending summer camp and would help to relieve some of the financial burden on mom. Student's father is not actively involved in his life and attending summer camp will give the student access to positive male role models as he continues to navigate his life. Mom has a child on the spectrum, so it is hard to find good day care. She is home with the kids until she can get services. That being the case, a kitchen table will help for the family's meals and for the kids to sit at and do activities. It will help the apartment to feel more like home.
