The children came to the attention of the department in 2017 due neglect by biological mother and the biological father who at the time was incarcerated. FR services were provided to the father and for some time the father was doing well, had overnight visits, but relapsed leaving the children orphans. The children have been placed with relatives since the time of detention with the case now being in the adoption process. The children are very excited to participate in Wildcats football & cheer for the summer, which the caregivers do not have the funds to pay for. Children would like to participate in the 2023 Wildcats football and Cheer season. The cost of football is $675, and cheer $300, for a total of $925 for both children to participate.

Mother died last year and then this year the uncle’s roof caved in due to the rain. The rooms had mold, and the home was in deplorable condition and all items had to be thrown away. The maternal uncle is legal guardian and he is doing the best he can with raising his niece and nephew (ages 11 & 12) but is struggling emotionally and financially. The children have behavior needs, emotional needs, and are grieving the loss of their mother. Maternal uncle is diabetic and recently had his toes amputated. Maternal Uncle was able to make some improvements to the home on his own, but needs beds and dressers for the children and is struggling financially. The request will help ensure the children’s needs are met and will prevent placement with unknown caregivers.
Support is being requested on behalf of the grandparents of a two-month-old infant, who are in need of a baby crib. These grandparents had been caring and planning to adopt their two granddaughters, when they were suddenly informed of the birth of their new grandson. These grandparents were not aware that their daughter was expecting and were shocked when they received the news from the hospital. In the middle of an emergency, the grandparents did not hesitate to take in their grandson at just one day old. These grandparents have committed to providing their grandchildren a safe and stable home, but in order for this to be possible, the child requires a crib. These grandparents have been looking for a second hand crib, but they have not been able to afford one. These grandparents have shared, that becoming the caregivers of their grandchildren, is not how they envisioned spending their golden years but that despite the challenges and sacrifice, they would not have it any other way. Support is being sought in order to purchase a crib in order for the infant to be allowed to stay in the home of his grandparents. Thank you so much!!!
The child is currently 14 years old and is currently placed with her mother. Her mother currently takes care of her and four other siblings. The child was diagnosed with Lupus and asthma and currently, she does not have a cellphone to make emergency phone calls to her mother from school in the case she has a medical emergency. Due to the fact that she, her mother, and the rest of her family currently resides in a shelter while facing potential homeless, the child's mother cannot afford to get the child a phone. This family does depend on the donations of others, and the child simply wishes for a phone to be able to contact her mother in the case of a medical emergency while she is at school.

The family consists of maternal grandmother and 14 year old granddaughter, who are struggling financially and are in need of paying household bills and buying food. A bit history of this family and why they are in need right now. In February 2021, the youth was adoptive along with her younger sister. On June 2021, 4 months after, adoptive mother died. The department gets involved. Adoptive father does not want this child and only want her younger sister. He allowed the child to stay with her biological maternal grandmother (MGM) , who the adoptive parents know but have no family relations. MGM wanted her granddaughter, but the problem was that MGM was residing in a senior leaving. Per Department policy we cannot approve anyone who resides in senior living because there are rules for those housing. Senior living do not allowed minors in those apartment. MGM was able to get a friend to help her move to a one bedroom apartment and her granddaughter was placed with her. Now 2023, a couple of month ago, MGM decided to move to a 2 bedroom apartment in West Covina. MGM is now barely making ends, she is struggling to pay house hold bills, rent and food. CSW is asking for 250.00 grocery gift card, as Grandmother said she is struggling and everything is so expensive. MGM stated she need money to buy groceries. Thank you for your help.

The child will be going to stay with a relative in the Lancaster area for an extended period of time (7 weeks) while the caregiver is out of town. The relative, who will be temporarily caring for the child, is renting a car to take the child to there 5th grade graduation in Long Beach, CA. We are requesting a $100.00 gas card to assist the relative with transporting the child to/from the graduation.
I am currently working with a single mom of two children: 8 yr old female and 1 year old male. Mother has been working hard to provide for family and cannot seem to make ends meet on just her income. During Covid, the family had many challenges which led to family being separated, however due to mother’s dedication the family is now together again. To ensure family safety mom needs to provide bedding for all members of family. Mom has successfully received a crib but needs a twin-size bed: frame and mattress for her daughter to sleep comfortably in their new home. It is critical for mom to provide bedding for crib, two pillow and twin sheets as soon as possible to ensure that she can continue to remain united with her family. Mom is working full time and is taking the steps necessary to see her and her family thrive in this new chapter of life.

There is a maternal grandmother (MGM) who is undergoing the RFA process. In order to meet the criteria for RFA approval, the caregiver is in need of a fence or mesh around her pool. I am advocating for this caregiver to keep the family unit together and to avoid the children being placed in foster care. The MGM has agreed to pay the remaining balance of $1095, and continues to need assistance of $1500.
Our clients (10 and 6) and foster mom were recently involved in a tragic car accident. The collision completely destroyed their car. Fortunately, our clients suffered minior injuries. However, foster mom is scheduled for surgery on her arm. Foster mom is the only caregiver in the home as her husband past away from COVID. Our clients lived through his passing and were definitely traumatized after this accident as well. During my last home visit, they shared they've been helping foster mom with cleaning and "cooking" sandwiches. Their bond is visually beautiful! Family is eagerly waiting for their adoption date.
Our clients and foster mom would benefit from giftcards to grocery or clothing stores to help with expenses through this hard time.

However, it was recently brought to case worker's attention that both the freezer and bottom sections are no longer working to keep food at safe temperatures. Case worker is unable to submit a request for assistance to DCFS, as it is the end of the fiscal year, and new requests will not be accepted until July. I identified the best priced item that can be provided to the family and placed the website to it within the notes section, although items from other sources are welcomed.

The children are in need of these items to have home cooked meals accessible and the mother currently has no bed, although the children do have beds. The family is closely bonded and has been participating in therapeutic services, parenting classes and other community based services. Any assistance would be greatly appreciated. Minor is a minor mother with a one-year old. Minor mother is currently detained from her Mom, and Mom is in reunification services. She is staying with a maternal aunt who has limited means. For these reasons, I am advocating for mother to have the needs identified met. The main reason I am requesting this need to be fulfilled is because in order to provide a safe, clean, healthy and comfortable environment for her family. It is essential to have these items as mother is having to eat out often, the house can be better organized and safer for the children. DCFS has assisted the family by providing rent assistance to the family. DCFS will continue to link the family to community services such as food banks, counseling, etc. in order to ensure the family continues in maintenance. For other items needed, I will explore other DFCS options for funding and in-kind donations. This need is being requested through Care Portal because unfortunately DCFS does not provide these items. This is a mother of two young children, a1 year old girl and 3 year old boy. The family currently has involvement with Child Welfare. The mother is currently in the process of addressing the concerns of the Department in the home in preparation for a Home Assessment to show a clean, safe environment for the children. The mother has beds for the children but needs sheets and blankets. If this family is blessed with the bedding for the children it will strengthen her home assessment and show the Department that the children with have warm, safe beds to sleep on. We are submitting a request for support in securing food assistance which would lift a huge burden from them and the three students overall wellbeing. A food voucher of $500. to assist the family in providing meals, this assistance would greatly help the family with the nourishment of their daily meals. We are assisting the parents in finding other resources that could also provide some assistance.

This is a family of a single Hispanic mother of five children 2 girls (12 years old and 6 years old) and three boys (3 years old, 17 years old and 16 years old). Mother is working full time, however, she is short on her rent for June. Mother shared that in a panic when she found out that the department was investigating her home, she went out and purchased beds for the children. Mother stated she was so worried that the DCFS would remove her kids because of the condition of their bedroom furniture. Since she spent $700 on the beds, she is short on her June rent. Mother is asking for assistance with paying $700 towards her rent for June.
Our agency is working with a single mother who is raising a child with special needs. The mother who has recently lost her housing earlier know this year and has been struggling to pay rent. The mother works a part time job and has been doing the best that she could to keep up with the rent payments and has been making payments every payments every time she gets paid. The mother and child is now facing eviction if she is unable to come up with 1,500.00 for back rent by June 1, 2023.
