A low income family of 5 is struggling to reunify with their children due to facing eviction. They are desperately in need of some rental financial assistance for past due rent to stop the eviction process.

Client has a severely autistic child who is now 5 years of age plus a 2 year old. Client has worked off and on and she had been working from September until March and then had to quit because of the 5year old behavior due to his autism. She recently moved because she thought it would be better if she was closer to her mother and sister. Her mother is who she can count on for helping her with the kids. FOB has not been a resource and her mother helps her when she can financially and always emotionally and physically her mother helps her with childcare, respite care etc. Client is requesting help with hygiene products (deodorant, lotion, female hygiene products), dish soap, Scott towels, toilet paper, pull ups/diapers and wipes for the 5 year old as he is not capable of being potty trained, laundry products and cleaning supplies for her home.

A 15 year old male was just able to move in with a paternal relative. KVC has helped the child obtain a bed and bed frame but the bed frame requires a box spring. KVC is asking for help obtaining a box spring for this family

I am a social worker working a family that is at risk of having their electricity turned off. Mom and Dad have three children and both work and have been able to afford their monthly bills previously, however, one of their children was recently admitted to CMH due to a traumatic brain injury. Due to this injury, Mom has had to stop working as pt now requires assistance with all daily living activities and requires 24/7 supervision for his safety and safety of others. As family has lost that income, they are now behind on their electric bill and at risk of having it turned off. The minimum payment to keep service on is $155.00. Jackson County Children's Division is seeking assistance in supplying a family who is in need of Uber gift cards to help them get around town & to attend their visitation with their 4-month-old son. It seems to be that having a vehicle and stable housing are the only barriers this family is facing that might be preventing them from reunifying within the time frame they would like to reunify in. Otherwise, they are doing absolutely everything & more to be able to ensure they are prepared for their little one to come home. This couple is doing exceedingly well on their progress towards sobriety and reunification with their son! The couple is 120 days sober from ALL substances and are actively participating in all services at this time! The couple is SO determined to reunify with their son, that they took it upon themselves to join other additional support groups on top of what they're required to participate in. They have even provided me as a caseworker with other resources within the local community that I hadn't known about, to then research and give out to other clients to help them! That is one of the many reasons as to why I enjoy working with this family so much. I am very proud of this couple and am hopeful that Care Portal will be able to find something to assist this couple in finding transportation vouchers. This family is needing assistance with rent/deposit for a new home due to the family needing more space for a child they are taking as kinship placement after a life altering accident. The child needs more space for the medical equipment along with the ability for them to work with nursing, physical, and occupational therapist in the home. Assisting with the rent/deposit will allow the family to move and settle in to their new home, and allow the child to discharge from the hospital after being admitted for over 6 months. Any assistance is appreciated.

A 12-year-old child is placed in a Department of Mental Health home to meet her needs as she has several diagnoses including autism spectrum disorder. This child has a history of causing damage to furniture items when she is escalated, in the past six months the child has broken two TVs in the living room. The DMH company has replaced the TV once already and we would like to try to alleviate the financial burden, while also finding a solution to this issue with a case. Please consider donating a television to allow the children in the home to enjoy their favorite cartoons and prevent future damage with the case that will be provided by DMH. A devoted mother has expressed her need for a one-seater stroller suitable for her infant. As a single parent, she is navigating the challenges of providing for her new child independently. When we inquired about how we could assist her, she shared that having a stroller would make a significant difference in her daily life.

Cornerstones of Care is working with a single mom who is trying to move into a new apartment with her 1-year-old son. The apartment is in need of being deep cleaned. The apartment is supposed to be nonsmoking but it smells really bad. Mom has a few different medical diagnosis and is unable to deep clean the apartment herself. She is needing to move into the apartment within the next week. We have contacted Servpro to do the deep cleaning but she is unable to afford the cost of it. Once she is able to move into the apartment her son will be able to live with her full time and be able to officially reunify.

During the home walkthrough, it became evident that this family, with four wonderful children sharing a bunk bed, would benefit immensely from having individual beds for each child. We strongly advocate for providing a bunk bed with mattresses for this family, ensuring that every child has their own space to rest and dream. Thank you for your support for this family.

Hello, I am requesting a $200.00 gift card for a sixteen-year-old young man who resides in a Department of Mental Health Residential Home. He is diagnosed with Autism and uses a communicative device to communicate with others. He enjoys bowling, basketball, going to Get Air Trampoline Park and shopping at Red Rocks Thrift Store. This money will positively impact this young mans life by ensuring that he can spend his summer participating in some of the activities he enjoys.
