There are two children on my caseload who have returned home to their mom a couple of months ago. The mom spent some time in a psychiatric hospital following the removal and after her discharge from the psychiatric hospital, she was living in a transitional house with other females. The mom was able to stay there as long as she needed and she was able to have her children returned home to her there. Her children did return home to her and they lived at the transitional house with her. The mom has continued to work her treatment plan with the agency and fully engage in her services. The mom has worked to get her and her children an apartment, which they recently moved into. I am requesting a bunk bed, two mattresses, two sets of sheets, two pillows, and two comforters for the family as the mother had no furniture or bedding prior to moving out of the transitional house. The oldest child has been sleeping on the couch and the youngest child has been sleeping with the mom in her bed.

Hello! I am working with a mother that is currently struggling with homelessness. Due to this, she does not have any security measures to keep her things safe, including keeping her medication out of reach from any children she is around, including her almost 1.5 year old daughter. Together we have discussed security methods that will work for her to ensure safety and protection, which would be a portable lock box that can fit in her backpack. I feel that this would help the mother feel safer with her items, as well as for the access of the medication and other items her child should not get into. In turn, that could aide in the mother being able to focus on other areas in pursuit of reunification.

There's a youth that has been in care for two years waiting for the court to adjudicate. At this time dad has been waiting without giving up with hopes to be reunified with his son. Dad is having car trouble to make it to visits and to his place of employment. Dad is in need of some help with paying for his car to be fixed and reliable. The cost of repairs is $400. We are hoping to get some help to ensure dad can keep his job and continue visits with his son.

I am requesting support to assist a 15-year-old youth attend summer camp as well as assist her foster family with obtaining a dining room table and a dresser. This youth is in placement with a relative caregiver who has two children of her own and a baby on the way. The caregiver is hardworking but cannot afford furniture for the apartment and has asked for support around obtaining a round dining room table with four chairs. The dining room is not very big this is why a round table was requested. The youth does not have a dresser and is using a box to hold some of her belongings. When asked if she would like a dresser, the youth responded that she does not like to ask for help but would really like a dresser to hold her clothes and put some of her belongings on in her bedroom. At the beginning of August, the youth will be attending camp. She has asked for a two piece bathing suit (shorts and a t-shirt material) as this is what she feels most comfortable in and a link will be attached to this request. The youth struggles with self-esteem and mental health and has really been pushing herself out of her comfort zone to be more involved in the community and get out and meet other peers her age. This bio-mom I serve had her two children returned home to her at the end of August. This mom spent some time in a psychiatric hospital after the children were removed from her. Once she was out, she was fully engaged in her treatment plan with the agency and fully engaged in all of her services leading to her getting her children returned home to her. The mom and her two children are currently living in a transitional home where the mom has resided since her discharge from the psychiatric hospital. The family is able to stay at the transitional home for as long as they need to, but the mom has been working to get them an apartment. The mom has asked for assistance with her car insurance bill for the month of July in the amount of $364.45 as she has been approved for an apartment and needing to pay all of her typical bills plus first months rent and a security deposit for the new apartment. This will help alleviate some of the financial stress that is on the mom as she makes this move to an apartment for her and her children.

This youth is 16 years old who was recently discharged from a residential center and is now looking for independent living placement as a form of permanency to eventually be able to live independently within the community. This youth is looking for a phone to stay connected with his adoptive and biological family, his friends, and professional supports. The phone the youth currently has does not have service and is unable to maintain regular communication with them. The youth would benefit from having a phone to stay connected with his supports, especially as he approaches permanency through an independent living program. This youth has a presence online through social media and has friends across the United States, having additional forms of communication will allow the youth to maintain regular contact outside of social media platforms.

Mom of 2 and grandmother of 1 is in need of a hotel for a week. She is currently staying at one paid through Wednesday. Mom works part time and has Section 8. She is actively looking for housing. The housing market is extremely difficult at this time. She has applications in and is waiting to hear back. She does not have a vehicle and takes the bus to and from work. Mom had a stroke a few months back and is only able to work part time. The grandson is under 1 year old. The children are 15 and 9. Even one night can help! They are not picky about where they stay. The rate listed would be for Woodspring Suites if they had availability.

A bio mom needs help with move in costs to reunify with her 6 month son that is currently in foster care. This is her only barrier for reunification. The total move in costs is approximately $3200. I appreciate your willingness to help reunify this family.

A 10 year old girl really wants to attend an overnight camp. The foster family needs helps with funding the cost of camp and needs help with camp supplies such as a sleeping bag, bug spray, sunscreen, towels, hygiene products, etc. Her foster placement is having a hard time supplying these items for the client. She goes to camp the week of July 28, 2025. Thank you for your willingness to help.

A teenager with significant mental health concerns is in need of a Bark phone for an increased sense of safety and wellbeing. In the case of a mental health crisis, a Bark phone I needed for this youth to be able to call their caregiver and/or supportive safe adults. This youth struggles with being safe online, so a Bark phone is needed due to the extensive parental controls the phone offers. Having a Bark phone will allow this youth to do everyday activities and be away from the caregiver while still having a sense of safety if a mental health crisis arises. This request is for a full year of the phone plan paid up front to ensure the youth and caregiver has adequate and full access to all safe features for this phone.

Mom of 2 needs assistance with her rent. She is in low income housing and works full time. However there was an error with her check and she will not get paid again for 2 more weeks. The rent is only behind one month. And mom has been trying to find a way to pay it before it goes to eviction.
