A mom is working to reunify with her 4 children. In order to do so this mom needs to be able to participate in Visitation with the children. Mom needs assistance with gas in order to be able to travel back and forth to visits. Mom is having to have 30-40 minutes each way to get to visits with her children. Mom has been working hard to complete all other services for DCS but needs assistance so that she can get to visits.

DCS has been involved since September of 2024. Children are ages 3 (male) and 5 (female) and are in the process of transitioning to a new placement due to another incident of domestic violence occurring in front of the children while in relative placement. The current placement has all appropriate services needed for the children other than the correct sized car seats. Both children need booster seats with a back.

The household consists of a Mother and her 10-year-old child. Mother has recently moved into some more affordable apartments. Mother and Child are currently sharing a bed. During the move Mother had to throw away the bed frame due to it breaking. Mother does not have any furniture besides the one mattress and a tv stand. Mother also does not have much food in the home. Mother would appreciate any assistance to ensure the home is appropriate and the child's food needs are met.

1. We are requesting payment for pest control services for a kinship placement . 8 children were removed and placed in kinship care with maternal grandmother. While doing the home visit at grandmother’s home, we noticed she had roaches in her kitchen. Grandmother was asked about the roaches, and she said she had been struggling with the roaches for a while but would treat them. Pest control is needed for the children to remain in the home with maternal grandmother. Grandmother is able and willing to keep all 8 children . The cost for pest control is 352.35. The invoice has been sent to MC Youngblood and she has approved for this to be sent to careportal. The pest control services were completed on 5/28/2025. The bill needs to be paid as soon as possible. Grandmother is working and should be able to pay for future treatments If needed. Since the children were an emergency placement, the pest control services were needed as soon as possible.

Mother could use assistance with receiving a pack and play for her 21 month old son and a crib for her 9 month old daughter. The family will need a fitted sheet for the crib as well to ensure safe sleep is being followed. Providing these items will ensure the children have safe sleep arrangements and improve their well being. Thank you!

Single mother of three children ages 6, 2, and 1 needs help with clothing. Mother works part time and is struggling to ensure all of the children's needs are met. The children have grown out of their summer clothes and shoes and you could help her by supplying the children with clothes and shoes that fit. Girl (6) is a 5 in clothes and a 1 in shoes, Boy (2) is a 3T in clothes and a 7 in shoes, Boy (1) is 18 months in clothes and a 6 in shoes. Please consider helping this family.

Relative placement that has a two year old child is in need of assistance regarding a toddler bed.

DCS has been invoked since April of 2025. Mother of child is deceased and father has not been in contact with DCS since the opening of this case. The child was previously placed in foster care in Lake County and just recently moved back to Marion County. The previous bed for the placement broke before the child arrived and FCM bought child an air mattress for the time being. Placement needs assistance with a new bed for the child that is more sustainable than an air mattress.

I am working with a little boy (age 4) who is growing out of all of his clothes. His mother made a comment that he's wearing clothes too small for him during visits. When FCM spoke with the relative care provider, they expressed a hardship currently. They cannot purchase the boy new clothes at this time and asked if DCS could assist. Unfortunately, we already provided a clothing voucher upon removal and cannot provide another one. If someone could purchase some outfits for this little boy, it would be amazing. He is currently supposed to be wearing 5T clothing, but most of his clothes are 3T and 4T. You can purchase a gift card to a store, or just purchase some outfits for this guy. He's not picky! Some specific needs are a light jacket, shorts, t-shirts, pants/jeans, anything else you like... Thank you so much for your consideration!

I am working with a foster mother and 10 yr old girl (turning 11 on 6/27/25) and she would LOVE an iPad for her birthday! This would also really help the foster parents with providing consequences and rewards for her behavior. Her behavior has improved greatly since being placed in the foster home, but this tool would definitely help. It would also give this girl the birthday gift of her dreams. Since being placed in foster care, she has seen neither her mom, nor her dad. Both of them have been very difficult to reach to get visits set up. Prior to being removed from her father's care, he told her that if she went to foster care, she would never see him again. He also told her that she would probably be sexually assaulted because she it cute. The little girl, obviously was afraid and displayed some challenging behaviors when she first came into foster care. However, her foster mom, who is very experienced, stuck it out and things have really improved. If you are able to fulfill this request, I would also ask that the iPad come with a protective case (she requested Hello Kitty, if you can find one!).

Single mother of two boys ages 13 and 7. Family has recently gone through a traumatic situation where supervision is needed at all times in the home. High monitoring is needed in each of the child's room to ensure their well-being and safety at all times. Family is only asking for 3 indoor security cameras where mom can observe from her phone. They will take any types of camera's.

Single mother with children of ages 15, 14, and 8. Mother is currently going through financial struggles and recently had a disagreement with other relatives who ruined her home. Mom and kids are slowly trying to clean up but are in needs of cleaning supplies. This will improve the children's environmental well-being and safety.
