The caregiver/maternal aunt recently obtained legal guardianship of the children after the tragic loss of their mother, following a murder-suicide committed by the younger child's father. The caregiver has expressed that the children are in need of clothing and shoes, as well as bunkbed in order for the children to have their own space in her home. These items will not bring their mother back, but it will assist with the healing process and ensuring the children are comfortable in their new normal.

16 year old youth has a hard time making friends and is introverted. In an effort to boost his confidence and self-esteem, we are requesting assistance with clothes, shoes, underwear and socks. Youth is growing rapidly out of his current wardrobe and caregiver, a non-related extended family member, has limited resources and cannot afford to purchase new clothes for youth at this time. Mother is a devoted, mother of seven children, ages 5-18. The case came to the attention of DCFS due to physical abuse, general neglect and caretaker incapacity. Ms. Washington works tirelessly to rebuild her life and reunify with her children who are not currently in her care. She is overcoming financial hardships while staying committed to her sobriety. She diligently works to complete Court ordered programs. In addition to working temporary and odd jobs to make ends meet to maintain stable residence in prearrange of reunifying with her children. As part of her progress, her older children now have unsupervised visits, but the home lacks essential furniture to sit and sleep comfortably. This is an urgent need as the Court reported that the children are fast tracked to return to mother's care on 05/07/25. This request will ensure children's safety as the residence will be clean and furnished with beds so they won't have to sleep on the floor. This request will strengthen parental capacity by providing emotional and physical care. This request will lessen her stressors and free up some of her time to spend with her children ultimately enhance the potential for permanency. This family of four includes mom and 3 children (14, 9, 5). Family came to the attention of DCFS due to physical/emotional abuse from father. Mother has relocated herself and the children away from father. As a result, mother is experiencing financial hardship without father's assistance. Family lives in a 2 bedroom/1 bathroom home and they are in need of a sofa bed that seats three people. A sofa bed will provide a more comfortable living and sleeping space for the entire family, thereby increasing the safety and general well-being of the children and reducing stress and anxiety for mother allowing her to focus more on her family and reallocate her already limited resources to other needs for her children.

I am writing to request support on behalf of Mother, who is currently involved with our agency and actively working toward reunification with her children. Mother has successfully completed all court-ordered services and continues to demonstrate commitment to her personal growth and family well-being by regularly attending outpatient services.

Mother has recently secured stable housing and moved into her new apartment. As she works to rebuild a safe and nurturing environment for her family, she is in need of basic household essentials to help her establish a functional and supportive home setting for herself and her children.

Providing Mother with this assistance will not only support her transition into independent living but also further her progress toward successful reunification. We believe that with this support, she will continue to build on the positive momentum she has achieved thus far.

The referred family is requesting assistance obtaining twin size bed for their child. The resource parent’s granddaughter is a 15-year-old with an intellectual disability. The resource parents care for her granddaughter on a permanent basis and is in the process of adopting her. The resource parent is attentive to her granddaughters needs and expresses her joy in caring for her. The resource parent is on a limited income and is an urgent need of a twin size bed for her granddaughter. The resource parent resides in a studio apartment with her granddaughter and is kindly request these items to continue proving a comfortable and adequate home environment. The family would appreciate any help and want to explore all the options to make the child as comfortable as possible at home.

The children are residing with a family member to assist mother her case plan goals. The caregiver needs beds to facilitate a safe and appropriate sleeping quarters for the three children. The items that are needed are one set of bunk beds, two twin mattress, and two twin comforter sets. Also, a toddler bedframe, toddler bed, and toddler comforter. All three children are female. Relative caregiver is on a limited source of income helping the family was unexpected and did not allow her time to prepare for the children arrival. Although, relative caregiver is having that challenge, caregiver is meeting the case plan goals/court orders. Furthermore, caregiver is committed to providing the three younger children with housing until mother can secure housing while the shelter, she is residing in does not have room to accommodate all seven children which is why the younger three are placed temporarily with family. The main reason I am requesting this need to be fulfilled is because the children need proper bedding to facilitate a safe sleeping environment. If this need is not meet for the younger three children, they risk the possibility of being taken back into DCFS custody and reunification services begin again.

This need is being requested through Care Portal because unfortunately DCFS will not provide the items in a timely manner and possibly take up to 8 – 10 weeks. The estimated cost of the need/s being requested is $1,185.00 dollars.

Mother needs a table with four chairs a sofa that seats two. Mother has been going through financial struggles after loosing her job and being evicted from the family’s previous home. She continues to actively look for work but has not been successful. The family continues to go through the ups and downs of the flare ups and emotional fluctuation of processing abuse. Mother has continued to actively support the child, and with her support, the child who has a known language delay has been able to process and learn to express herself more using her expanding vocabulary. Mother ensures that both her son and daughter attend all Wraparound services. The family was left homeless after mother could no longer work to care for the children as she was unable to leave the home where the abuse occurred. The family ended up being evicted and later spent months awaiting their turn for a shelter. Mother was unable to keep any of her hard earned belongings and now that she was able to find a new home she is need of the noted items to finish furnishing their new home. DCFS is requesting a table and a sofa, in order for the family to be able to have these essential items in their home. This need is critical to ensuring that the family is able to bond and feel comfortable in the home. The estimated cost of the needs being requested is an estimated amount of $400 for a sofa or love seat and $350 for a dining table that seats four. For an estimated total of $750.00.

I'm reaching out on behalf of a resilient young man currently navigating life as a Non-Minor Dependent (NMD), working hard to build a future for himself in the trades. Despite limited resources, he shows up every day, borrowing tools from co-workers just to get the job done. This dependence, though met with kindness, leaves him vulnerable and often unable to work independently. With a basic set of tools of his own, he would gain not only the ability to fully perform his duties but also a sense of pride, stability, and ownership in his growing career. Your support would not just provide equipment—it would empower a young man striving to stand on his own feet.

"The non-minor dependent (NMD) came to the attention of DCFS and the Court when she was in the 4th grade. She was removed from both parents due to their severe mental health challenges and homelessness. A church member later became her legal guardian and cared for her until she graduated from high school and enrolled in college.

Unfortunately, due to her own mental health struggles, she was unable to complete her first semester and withdrew from college. She temporarily returned to her former legal guardian’s home, where she has stayed for the past 10 months. However, the guardian’s family is now facing their own challenges and has asked her to move out by 6/30/25.

The former guardian helped her find a small, rented room near their workplace and continues to support her transition to independent living. Although the NMD is still struggling with mental health and independence, she is motivated and willing to work toward full self-sufficiency over the next year.

She is in need of a twin-size bed and other basic household items to settle into her new space. "

A 16-year-old foster youth was recently placed with his paternal grandmother after years of not knowing each other. Despite the sudden placement, the two are working hard to build a relationship and create a sense of stability.

The grandmother, who is on a fixed income and currently receiving no financial assistance, is doing her best to support him. The youth arrived with only a few items of clothing, most of which no longer fit. He is in need of a new wardrobe and would greatly appreciate gift cards to Tillys, PacSun, or Zumiez. Even donated clothes in good condition would help. His sizes are: XL for undergarments, shirts, and shorts, and 38x32 for pants.

In addition, the youth needs a full-size bed frame, mattress, bedding, pillows, and a dresser, as the home was not prepared for his arrival. Since the grandmother does not have transportation, a bike and bike lock would also make a meaningful difference, helping him get to school and connect with his new community.

This young man has faced multiple placement disruptions, and providing some of these essentials could help him find the stability and comfort he needs in this new chapter of life. Any support—whether new or gently used—would be deeply appreciated. Thank you for considering this request.
