The family consist of three children ages 3 (female), 15 (male), and 17 (female) years old. Due to the father suffering with a severe substance misuse addiction and not wanting to engage in treatment, the mother asked him to leave the home. The mother currently works from home full-time to support her children. Due to the father leaving the home earlier this month, the mother had to pay all of the household bills in the home on her own. This caused her not to be able to pay her car-payment for this month. Although mom is working, she is now the only source of income and provider for her family. Assistance with her car payment will help alleviate the stress of her vehicle being repossessed.

The mother is in need of assistance to pay the balance of her light bill.

The client is a 35-year-old mother living in a shared apartment with her partner and their one-month-old baby. The client reports persistent emotional distress and low mood, often feeling overwhelmed by her circumstances. Breastfeeding has been challenging, as her baby is sensitive to milk. The lack of access to transportation increases her isolation and limits her ability to connect with the community. Through case management and access to tangible resources, the client can regain stability, autonomy, and social connection. Support in the form of baby care items; including Similac Alimentum Hypoallergenic Formula (Purple Can), 6-month-sized clothing, baby towels, baby wipes, 6-oz baby bottles, an electric breast pump, and a crib cushion—would directly address the baby’s care needs. Stroller and baby carrier would support the client’s mobility and reduce her isolation by enabling outdoor access and community engagement. To promote the mother's emotional well-being, self-care items such as a relaxation candle, facial mask, and lavender shower soap are recommended. ****NOTE: “This request is being submitted by the Center for Family Services of Palm Beach County on behalf of the client. The family needs assistance with groceries until the grandmother can get the account straightened out.

She gave birth to her youngest child just over a year ago and now requires a double stroller to support her daily travel and caregiving responsibilities. The client is employed part-time and has expressed financial difficulty in purchasing a stroller at this time. One of the client’s therapeutic goals is to reduce stress, and engaging in regular walks has been an important self-care strategy for her. A double stroller would significantly enhance the client’s ability to care for herself and her children. She needs assistance with the FPL bill. This assistance will ensure the family of four are able to continue to have electricity in the home.

Mom is a single mother of two. She has no car. She replies on her neighbor to give her when he is available. Mom has a declining mental health. She needs to go to a psychiatrist but has no transportation. She needs a bus pass in order to go to appointments. This assistance with ensure Mom get the mental health help she needs in order to best support her kids.

Mom is a single mother of three. She works full time to support her kids. She currently does not have a car. She relies on her ex-husband to help her. Whether she needs to pick up the kids from school or get groceries. She needs help with a bus pass. The bus pass with help her in order to not reply on her ex-husband. As well as being able to pick groceries or the kids from school when needed.

She previously lived with grandma but has now move into her own place with the kids. She is requesting assistance with her FPL. This assistance will ensure the family of five continue to have electricity in the home

The mother is a victim of domestic violence with two minor children ages 2 and 1. At this time, she is in needs of pampers for the children. The two year old wears size 7 and the one year old wears size 6.

The mother is also in need of two toddler beds for the children to prevent co sleeping and possibility of SIDS
