Client is a single, working mom of four children, her youngest being a nine-month-old baby boy who is approximately 20 pounds. Client is in need of a highchair for her baby, as he is struggling with the transition from formula to food. Client would greatly benefit from a highchair so that she can multitask in feeding her baby while helping care for her other kids.

Per First Care policy, equipment must be brand new. Client is wanting a general highchair that can grow with her baby as he gets older and bigger, preferably one that is convertible and adjustable.

This mother (43) lives with her two daughters (2 & 8 years old) and is requesting support due to several factors. This mother is currently pregnant and is navigating her health throughout this pregnancy. In addition to that her 2-year-old daughter has significant special needs, and this mother is navigating ensuring that her daughter is attending all of her medical and therapy appointments. Her supports are limited, as her entire family resides in another country. This mother does not drive and works as a fulltime PCA for her 2-year-old daughter. This mother would like to work towards ensuring that her 2-year-old daughter has the necessary supports in place and that she is attending her necessary appointments related to her pregnancy. Additionally, this mother is working towards connecting with community supports to assist her and her children in being able to meet their basic needs moving forward.

Spanish speaking mother of low economic means is requesting a full size crib and mattress for her 1.5 year old daughter. Per our organization's policy, we accept new items only.

Client is a single mom of two and is currently in need of a stroller for her 19-month-old son. Client explained he is quickly outgrowing his old stroller as he is 25 pounds, and is in need of one that can grow with him. Client would like a 2 in 1 convertible toddler stroller, preferably one that is also collapsable for being on the go. Client would greatly benefit from a new stroller as she is currently balancing work, school, and parenting; and would have one of her biggest needs met by receiving a stroller.

We are requesting support of a family that we've known for years and has been a pillar in the community. Specifically they need repairs completed on three windows that are currently unsealed and leaking water into the home, and probably collecting mold. The youngest child is an infant and has already had health concerns, and the next child has severe asthma that flares up regularly. They've tried different ways to seal the windows and have only been able to come up with a temporary solution. We think the windows getting repaired will greatly improve the health and well-being of the family, and reduce their stress knowing their kids aren't being affected by the moisture coming in the home.

Client is a single mother with a 13-month-old child. Client is in need of a new stroller for her child as he is quickly outgrowing his current one. Client is struggling financially, and would greatly benefit from receiving a stroller so she can use her income to cover basic necessities and be able to take her child around more freely. Client is specifically wanting a convertible baby stroller that is reversible and foldable and can grow with her child.

In need of a bunk bed to help the father reunify with his children. The children are two young children who love their father and want to be with him. He struggles with addiction however, he has obtained sobriety over the past two months which is a huge success for him. Any help would be appreciated.

This student's family is at risk of losing their housing because of increasing prices and Grandma (guardian) losing her job. She is struggling to get unemployment or any other support. She works incredibly hard to take care of her grandson and who has a high level of needs. As the head of household for a house of 6 people, she does a huge amount of work.

Grandma just finished school and is hoping to find a job soon, but right now things are very difficult. The housing instability is causing our student increased stress during the school day, which is greatly impacting his ability to control his emotions at school and learn. He has gone through a huge amount of trauma in his life, so any additional stressors cause him to become very disregulated.

Household members: Grandma, Great-grandfather, 13 year old, 20 year old, 8 year old (our student), 11 month old.

I gave grandma a financial assistance resource, but support in other ways would be incredibly appreciated to help this student be more successful in school and for the family to maintain their housing.

Grandma indicates the major need is assistance with food. Feeding a family of 6 is very expensive. Any gift card or delivery of groceries would be appreciated. (Walmart, Cub, etc). Thank you.

This is a two-parent household with five children under the ages 6 and under. There is one child with specialized needs and other children who are participating in the evaluation process to assess what additional needs they might have. The father of the children has had a loss in his family and had to travel out of the country to address this loss, and this put additional strain on the household due to loss of support for the family both physically and financially. In addition, their youngest child is an infant therefore the mother of the children has only recently been able to return to work to help provide for the family financially. These children would benefit from the following support to enhance their learning, development, and safety.

There was not an option for a crib mattress protector, so I had to enter it as twin, but we need a crib size for the baby. Due to space if the dresser could be a taller type rather than the shorter/long it might work better in the family's home.

Client is in need of a play mat/activity pad for her daughter who just turned one month old. Client has had some support from family but is not working and is mostly home alone with her baby. The play mat/activity pad would greatly improve baby's wellbeing as she is growing and developing, and allow client to prioritize making other necessary purchases.

I am submitting this request as a school social worker on behalf of a family that I have been working with throughout this school year. The mother is a single mother, and has been experiencing a lot of financial stress due to employment struggles, and costs to transport her children to school. They open enrolled to our home district as one of her children was experiencing a lot of racism at their other school. They were looking for a fresh start and a new setting, but having the responsibility of taking care of two young people, the cost of rent, and providing transportation via her car or by Uber due to her work schedule has taken it's toll, and they are in genuine need of support from others to get through the remainder of the school year. I am asking for people to please help cover part of a months rent to provide some financial breathing room for this family. We appreciate all that consider reviewing this request. Thank you very much.
