Three school-aged children need support with obtaining three chromebook devices to be able to complete distance learning tasks from school. The children also need support with obtaining one 4/5 drawer plastic storage container to store their clothing and personal items. The mother of the children is currently hospitalized for possibly long term, and the children are residing with a temporary caregiver (family friend) outside the family home. As the children are not able to be transported to school now due to mother's hospitalization, the school has authorized distance/online learning temporarily, and the school did not have devices to provide the children with, and that is the reason why support with obtaining the devices is being requested from the CarePortal Community. The children also need the 4/5 drawer plastic storage container to have a place to store their items. If the children are supported with obtaining the items, DCFS will have confirmation that the children are keeping up with their educational requirements and that they are residing in an organized environment, making it possible to maintain the children in their current placement and out of foster care. The children are currently safe and thriving in their current placement with the family friend, that they known all throughout their life and it would be beneficial to maintain them in the individual's care during this time. As the address cannot be provided, the assigned worker can arrange to pick up items.

Maternal Uncle began caring for his 2 nephews after their Legal Guardian, maternal Grandmother passed away. The transition was difficult, but the maternal uncle has been a steadfast and loving caregiver. The maternal uncle moved the boys in with him, and his fiancée at the time, and they were able to provide the boys with a loving home environment along with the emotional and social support they needed to thrive. Maternal Uncle and the children were recently displaced from their home due to the relationship separation between maternal uncle and his fiancée. The maternal uncle was able to find a new place to live, however their belongings in the home are sparse and the family is in need of a dining room table. This will allow the maternal uncle to continue to provide the boys with a nurturing home environment. This will allow them the opportunity to have furniture in their home along with a place to sit and share meals together in order to strengthen their bond.

Mother of 4 children (ages newborn, 1, 2, and 4) is currently seeking assistance for a manual breast pump. Mother recently gave birth in June to her 4th baby. At that time, mother was provided with a breast pump from her physician but during a transition from a shelter the breast pump was misplaced. Mother attempted to request a new one from her medical provider but results have been unsuccessful. Mother is in desperate need of a breast pump to maintain her milk supply to support the growth and development of her new baby. Unfortunately, due to the rising cost of living, the family is unable to purchase a replacement pump due to financial stressors and limited resources. Support for this family, will reduce stressors which can often impact abuse and neglect for many families and assist mother in meeting the basic needs of her newborn baby. Her child is not in her care in large part because she was a very young mother who had very little support from her family. Her child was removed from her care in 2022, around the same time she was removed from her mother. She has always maintained visits with her child. She has unsupervised visits in her foster home with her child but these visits would be better if she had better baby items for her child, who is nearing 2. Caregiver’s baby is in need of a walker and a carrier with a total cost of $140.00. This assistance will eliminate caregiver’s stress levels as she will be able to ensure minor’s needs. The infant can benefit from the walker for developmental purposes to learn to walk and the carrier will support the mother with make transportation on public transportation more accommodating. With obtaining these essential items, the mother can demonstrate to DCFS that she is assuring to support the child with developing gross motor skills.

A single mother and two children are in need of a gas stove, a microwave, and clothing for the children. The seven year-old girl uses size 8 tops and bottoms, while the six year-old boy uses size 7 tops and bottoms. The family's stove is very old and is no longer working and with a functioning stove the family will have access to home made healthier meals. The family also is in need of a microwave to be able to prepare meals that the children need. The children have out grown their clothes and are in need of either new or gently use clothing that fits them. The mother is temporarily not working due to health issues and struggling financially making it not possibly to obtain the items on this request. The children's father is absent and has not provided financially for the children, although the mother is pursuing child support assistance to be received in the near future. The family has been making progress in participating in supportive services with the Shields for Families Family Preservation Program to improve the family functioning in many aspects of their life. The goal is for the mother to provide a completely clean and sanitary home for the children and to provide them with essential items, such as clothing and other basic needs. This family consists of a single mother who is caring for her three children that have a variety of special needs. All the children are non-verbal. The oldest child is on the spectrum of Autism and has more escalated and extreme behaviors that mother is trying to safely address. The child has behaviors that include eloping from the home out of the front door, and playing roughly at the second story apartment windows that has caused the window screens to tear. It appears that the child gets over stimulated when he is watching neighboring children play outside at the playground, so he is pushing onto the window screens. Mother has placed work orders to have the window screens fixed in the past but the children continue to bend the window frames and tear the mesh. Mother is the sole provider for the children and father is not present, therefore mother is unable to work to provide continuous care to the children. We are requesting the purchase of window safety guards to prevent the children from falling from the second story apartment windows. The CSW has located window guards on Amazon. See attached link: The following window guards are needed: 1 order of 61.41”-86.60” AND 1 order of 36.23”-61.40”. A TOTAL OF 4 WINDOW SAFETY GUARDS ARE NEEDED SO THE REQUEST HAS BEEN BROKEN INTO TWO SEPARATE REQUESTS. The building maintenance crew will install the window safety guards.
