The family has an open FM case and consists of the parents, and 2 minor children, 17 and 10 years old. The family has an open FM case due to substantiated allegations of general neglect due to hygiene concerns and domestic abuse. The mother is currently unable to work due to health concerns and it awaiting surgery. The child has developmental disabilities and his behavior can be challenging to manage. The father is in an out of the home but is not working and helps care of the children and take them to school and other appointments. The family is struggling financially due to the mother not working and the children are in needs of tennis shoes and warm winter clothing. The assistance would alleviate stress for the parents and would make the children happy to have new clothes and tennis shoes. The mother shared that the child has wide feet and struggles to find tennis shoes that fit his feet. Mother shared that the child also needs a new backpack and school supplies.

They reside in one room in a house rented from a friend. The father works as a laborer while the mother is actively looking for employment but has encountered barriers in finding something consistent due to her being undocumented. The mother currently attends English classes in hopes of improving her communication skills. They are receiving support and assistance from a local family resource center. The services include food distribution, financial coaching, counseling services for the parents and children. This will also be the family's first holiday away from their family in Guatemala. They were living in an apartment but there are some issues that do not allow them into the apartment for at least the next two days. The weather has turned cold and the family has nowhere to go. Mother works Tuesday-Friday from 3:00pm-9:00pm across the street from the home due to needing to also be close by due to maternal great aunt being elderly. Mother does not have a working refrigerator and is using a mini one to store all her food, she is unable to purchase anything due to her very limited income. Currently they are unable to store much in the fridge due to it being extremely small. Mother is also struggling with buying groceries and on occasions goes to food banks to obtain food.

Parents were able to successfully obtain their own place, but need assistance with some furniture for the children to be able to start their overnight visits and to be able to return home to a safe environment. Mother recently received placement into a new home, but unfortunately due to rapid changes mother does not have funds to purchase twin beds for her children. Parent is in need of four twin beds and a twin bunk bed. Due to current status, children sleep on an inflatable mattress on the floor. Mother hopes to receive beds during the holidays to bring a sense of joy into her children's life's after these difficult times. Mother is working on utilities and additional furniture for their house. Receiving four twin beds and bunk bed would help mother with additional support in stability. The help will provide another level of comfort and support for the mother and family. Receiving beds will assist family to have a safe and warm bed during these cold nights and will help the house feel like a home to them. Working with a parent who is part of the DCFS workload, the worker reached out for a bit of assistance with his mother's situation. We helped create a plan that ensured the mother/minor's safety and well-being. The family is currently homeless and living in their car. Mother has stage two cancer and custody of her children. DCFS is currently in the process of securing stable housing for the mother. However, it will be two more weeks before the family can move into their home. As a result, the worker reached out for assistance in any way she needed, such as paying for a hotel room. DCFS is able to provided coverage for a week. And we are wondering if someone might be able to cover the second week which would be 11/28/2023 through 12/05/2023. The cost is $99 per night, a total cost of $693 for 7 nights. She is currently at the Bellflower.

The father has been applying to new positions as he currently has two interviews scheduled. The father has been doing small jobs in the meantime so as to continue to have some type of income. The father has had the support of his family whom have provided food and other utilities for him. The father has also reached out to local churches and or free produce (fliers) the undersigned has provided him with. The father is in need of $500 for his November rent. If granted the funds to help with one month’s rent, the father will be able to catch up with rent and continue to make his payments afterwards, thus enabling the father to continue to progress, be able to provide stability for the child, and continue with liberalizing the father’s visits to overnights by the end of the month and eventually reunify.

Hello, I am making this request for a family of 9. This family recently reunited and the Department is working on stabilizing the family. The mother was provided housing by the Family Reunification Program, and is in great need with assistance to furnish the home. Previously, the mother and the children were living with the maternal grandmother, which was causing a lot of discomfort, due to there being a large amount of people living in the home. The children have stated they are thrilled to be living in a home for themselves, and are looking forward to making their new home, feel like home. The mother is currently employed, but is only making enough to support the children and additional bills. The children's ages include: 15, 14, 13, 11, 10, 9, 8, 5. The mother is hopeful she can have a large sectional couch, in order to be able to sit the children during family time.
A child that is five has been reunified with his father. He and the father sleep in the same room. The father is currently rebuilding his life as he now sleeps in a twin bed. The family is requesting a toddler bed and mattress for the youth so he and the father can sleep comfortably.

Good afternoon, I am the Case Manager of a student who is in current need of a new bed frame, and a mattress. The mother's finances are currently restricted, and the child reached out to me, in an attempt to alleviate the mothers' financial strain. The child we service is receiving mental health services. The child has excelled academically over the course of the academic year, but the disturbance in her sleep is causing a decrease in her academic standing. The child cannot make use of her current bed any longer, hence it is broken. The family's financial restrain has made it difficult to purchase a new bedframe, and a mattress for the child. I have spoken to the family and determined that the child will benefit from having a place other than her broken bed obtain adequate rest. Due to the clients size, she needs a bigger bed, in her case a queen bed. The child and her mother are consistent with reaching out for support and our organization is devoted to helping her and her family reach their overall gals. Mother was a foster youth as a child. The only barrier at this time to Mother’s reunification is to secure housing for herself and the children, which she has now successfully completed. Now mother is in need of support for the purchase of a refrigerator and a dining table for the home. Once she has secured all utilities and appliances, DCFS can walk on a request to reunify the children with Mother.

We are asking for financial assistance for a mother, 31 years old, who is ill and placed in a nursing care facility due to her critical medical issues. Mother is blind in one eye and has 9% kidney function. Despite her medical condition, Mother continues to work on reunification. The mother visits with her children at the nursing home facility, but due to her medical status, she is unable to work and has zero source of income. Her children are older, 14 and 16. We are requesting a financial donation for a mother to use on her visits with her children. The mother would like to be able to offer the children a meal, go out of the facility, buy them something, or purchase things she and the children can use during the visits. Any financial assistance received will be used to strengthen the bond and quality of the visits with the children, while they still have their mother. The mother has a parent partner assigned who is helping her come up with activities she can do during the visit with her children. It is unknown if the mother will receive a transplant donation, so the time of the visits is crucial for these children.

She is doing her best to support her family but due to appointments related to the case, court case, medical and other services that DCFS is requesting the family to do, the mother has had to take a lot of time off, and since she just started the job she has no paid time off. Mother is of need of Rental assistance of $1671. The child who is 1-year-old attends occupational therapy and the child who is 3-years-old receives Applied Behavior Analysis therapy. Mom’s 13-year-old daughter also attends therapy. Currently, mom’s car is not working. Mom stated that she took her car to the mechanic and mom needs a catalytic converter for her car to be repaired. Mom does not have the funds due to working part-time. Mom has been struggling to take her daughters to the necessary services to meet their needs. Local Family Resource Center has assisted mom in saving money by providing her with basic needs such as food, a bus pass, diapers, and a store gift card. Mom has been getting rides from her neighbor upon availability. Mom explained that she runs late to the appointments or must reschedule due to not having consistent transportation. Due to mom’s car not working and not having the funds to pay for her car, mom has been feeling stressed and her blood pressure was high, so mom had to go to the emergency room and was told to rest and take days off work. Mom has not followed doctor’s orders due to not being able to afford missing work to be able to provide for her children. By being able to have the catalytic converter repair, the mom will be able to have her own functioning vehicle to transport her children to their necessary appointments and reducing the high stress she’s been feeling
