Innovation to Elevation Daily Living is an Independent Living Program for teenagers aging out of foster care. This program helps them find an apartment and teach them the skills they need in order to be a successful, independent adults when they turn 21.

We had an influx of residents and are trying to grow our team, but at this time, we do not have enough staff to transport all of youth to their jobs. None of them have transportation and this is interfering with them being able to maintain a job to have a stable income. It would be greatly beneficial if you would be willing to provide uber gifts cards for this need.

A family of 7 children, 5 boys and 2 girls ages from 5 to 15 have been re-unified with their biological mother and are in need of basic furnishings to assist in their transition. The mother has provided the majority of the items necessary to furnish the home but is still lacking a full-size bed for one child and comforter sets for all 7 children. The mother will benefit from these items so that she can use her funds to purchase additional items needed for the children as well as back to school items.

This family has been served previously through CarePortal. At the time, the priority was getting the HVAC and the ceiling fixed. Now that this has been fixed, we have noticed other needs. This is a maternal aunt who has custody of her three-year-old niece and thirteen-year-old nephew. The client has maintained custody for several years. The client has no family support who lost her mother (grandmother to children) last year.

She currently works from home, and she has an eighteen-year-old who is on the autism spectrum as well. She is seeking respite care maybe a mom's day out church program? Clothing, food assistance, employment assistance are high on her needs list as well. She recently had problems with her HVAC system which caused damage to her ceiling from leaks and caused her water bill and power bill to become high. We are wanting to help her to maintain the children in her home and she is trying but is overwhelmed and needs help with utilities, clothing for children, and food. Thanks.

The family consist of three smaller children currently residing with their aunt and she is doing the best she can with raising the children while their mother is away at this time. The children are in need of some clothing for the sizes for two girls and 1 boy. The 7 year old girl needs sizes 10/12 and size 2 in little girl shoes. The other little girl who is 3 years old is in need of a size 5/6 and in kids shoes a size 8. A little boy who is 2 years old and he wears a size 2t-3t and in shoes he wears a kids 6. Also, the aunt is in need of some gas cards to help commute the children back and forth to school and summer camp and also the 7-year-old girl is in need of a booster seat, the 3-year-old is in need of a booster with a back, and a car seat for the baby boy. The aunt is also in need of some financial assistance with one of her utility bills as well.

The family consist of a teen mother (17), her child (1 years of age), and her mother. The family does everything they can to make sure they can consistently help each other out the best way they can but, they are in need of some things to help strengthen the family to be comfortable. The baby boy who is 1 years of age is in need of some clothing, and he wears a 1t-2t in clothing, a 4c in shoes, and in pampers he wears a 5/6. The mother is 17 years of age, and she wears a size 4 in grade school shoes, and Junior 0/1 in clothing. The mother of the 17-year-old and the teenage mother are both in need of gas cards to help them commute back and forth as well to work and to care for the child.

Grandmother took custody of twin boys and granddaughter that is 8yrs. Grandmother is not working due to having young children. Grandmother just moved into a larger apartment to accommodate the children. She is now in need of these essential items to be able to care for these children properly. Thank you!

Grandmother is trying to prevent her four grandchildren to enter foster care. She was currently fighting breast cancer and is in need of support to keep her grandchildren. This will mean the children would be able to stay with family and prevent to enter foster care. Thank you so much for your support for this family.

A therapeutic foster mom has opened her heart and home to a special needs non-verbal 15-year-old boy. To help continue to care for him, he needs new bedding - including a full mattress, box spring, mattress cover (waterproof), pillows and pillowcases. The bedding he currently has become soiled by him and needs to be replaced. - It would be helpful to the foster parent to have more than one set of bedding as well as more than one mattress cover that is waterproof.

The mother is a single mother of three teenage autistic children (17. 15. and 13). She is struggling financially to meet the needs of the family. She has recently obtained housing through a friend and is in need of a stove and refrigerator to help provide basic food needs for the family. Family has no other support or resources. The children are also in need of three dressers.

This is a single mom that just got a job. She has two little boys. They are in need of a sofa. The sofa will provide an area for them to sit together and spend some family time together. The sofa will contribute to a sense of home. When this need is met they and others can come together and enjoy being comfortable.

I am reaching out to request assistance in obtaining new twin size mattresses for two foster children in our care. Unfortunately, due to frequent incidents of bedwetting, the current mattresses have developed an unpleasant odor and need thrown away.

We are hoping to ensure a comfortable and healthy sleeping environment for the children. The bed size is twin and one of the children is a female and the other is a male.

Grandfather wants to take care of his grandson the best way that he knows how. The child needs a bed to sleep in to stay in this placement. Helping this grandfather to keep his grandson is the best outcome.
