The patient is a young mother currently experiencing homelessness. She is living in her car with her five-year-old daughter and is expecting another child imminently. Due to her circumstances, she has been unable to access consistent meals, prenatal care, or provide her daughter with the necessary support and stability. She has no assistance from family or the father of her unborn child. The patient is feeling extremely scared, isolated, and overwhelmed. Providing her with immediate support would not only address her critical needs but also offer her the encouragement and stability necessary to move forward, reassuring her that she is not alone in this journey.

The 17-year-old female youth and her boyfriend (who aged out of the foster care system) are expecting their first baby, 5/30/2025. The couple are diligently working to make plans for their family by continuing their education, taking parenting classes, and learning independent living skills. Through their support system, they have acquired most of the essentials needed for their baby and have set up the nursery. The monitor is important to the expectant parents as it provides emergency medical alerts including co-sleeping behaviors. They also need stroller with a bassinet. Suggested LINKS provided below.

The mother, has 3 girls (14, 13, 12) recently obtained housing for her and her children via Broward County Housing Authority. The mother's case initially came into care due domestic violence. Since the case's inception until now the mother has made some extremely significant behavioral changes and has increased her parental protective capacities drastically for the benefit of maintaining to care for herself and children without the father. The mother is a single 36-year-old African American female that is trying her absolute best to provide for herself and children alongside trying to be maintaining a safe environment for her children. The mother indicates that she is really in need of some assistance with obtaining bikes for her children as she is unemployed and unable to afford them with the little assistance she receives. The mother indicates that she is trying her absolute best however she could really benefit from a lending hand. The mother is indicating this will encourage the children to attend school regularly and improve their grades. 3 bikes are needed so the children can get to school. (2) 24-inch bikes and (1) 26-inch bike. These bicycles will help the family TREMENDOUSLY!!

We are reaching out on behalf of one of our high school seniors who is currently experiencing a significant season of grief and transition. Her family recently lost their beloved matriarch—her grandmother—after a long and difficult battle with cancer. The student, along with her mother, younger brother, and uncle, are all deeply grieving this loss. Her mother, who is the sole provider for the family, is working full time while also navigating the challenges of moving to a new home and caring for her children. In this time of emotional and logistical upheaval, preparing daily meals has become overwhelming, and the family is in need of food support. Additionally, our student has a specific and urgent need: a durable, heavy-duty queen-size bed frame. She currently has a queen mattress but has broken multiple frames in the past due to weight limits. She is actively working on improving her health, but in the meantime, her broken bed is making it difficult to sleep properly, which in turn is affecting her emotional wellbeing and school attendance. Working toward the goal is reunification, the mother has made progress towards the goal. Little girl 1 y/o is one of three children. The mother’s visits have recently been changed to unsupervised. The mother is allowed to sign the child out from day care for her visits. It was reported by the daycare that the first day the mother signed the child out she was observed walking with the child in the parking lot without holding the child’s hand. The staff informed the mother to hold the child hand as there are cars going in and out constantly. The mother reported that she needs a stroller for her visits/ and upon reunification. The mother reported that she only works part time at Publix and does not have the funds to afford a stroller as she also must pay child support with her dependency case which she purchases clothing/ diapers and wipes for the child. The mother would be grateful for any additional helps she receives.

Subject: Assistance Request for Family in Need: We are requesting assistance for a hardworking family facing significant challenges. The primary provider, his guardian aunt, who runs a veterinary clinic where some family members work, is currently undergoing cancer treatment and has been unable to manage the business. As a result, the family has struggled with consistent work and financial stability.

Aunt has custody of our student since his bio-mom struggles with substance use and is unable to care for him. He is a senior, has shown incredible resilience in the face of adversity, overcoming bullying, food and clothing insecurity, and transportation difficulties. Despite these challenges, he has remained dedicated to his education and will proudly graduate next month.

We would love to bless him with a new outfit for graduation as a well-deserved reward for his perseverance and hard work. While the request form lists children's clothing sizes, he is 17 years old and requires men's sizes. Any assistance would be greatly appreciated.

A single mother of three is currently is maintaining her stability with very little support from the fathers and family. The mother is responsible to transport her children to school, she also uses her vehicle for her daily needs. The family recently experienced issues with the mother of the car. The mother is able pay for the car repairs, however the mother is in need of $70 to have the car towed to the mechanic. This is a urgent need for the family since the car is used to transport the family.

A mother of 5 has maintained her stability and the family is thriving. To avoid financial hardship, the mother is seeking assistance with purchasing a mini crib for her infant, The baby is currently sleeping in a bassinet, however she has outgrown the bed and is in need of something bigger. The mother has no preference, she is open to accepting what is available
