16 year old youth has a hard time making friends and is introverted. In an effort to boost his confidence and self-esteem, we are requesting assistance with clothes, shoes, underwear and socks. Youth is growing rapidly out of his current wardrobe and caregiver, a non-related extended family member, has limited resources and cannot afford to purchase new clothes for youth at this time. Mother is a devoted, mother of seven children, ages 5-18. The case came to the attention of DCFS due to physical abuse, general neglect and caretaker incapacity. Ms. Washington works tirelessly to rebuild her life and reunify with her children who are not currently in her care. She is overcoming financial hardships while staying committed to her sobriety. She diligently works to complete Court ordered programs. In addition to working temporary and odd jobs to make ends meet to maintain stable residence in prearrange of reunifying with her children. As part of her progress, her older children now have unsupervised visits, but the home lacks essential furniture to sit and sleep comfortably. This is an urgent need as the Court reported that the children are fast tracked to return to mother's care on 05/07/25. This request will ensure children's safety as the residence will be clean and furnished with beds so they won't have to sleep on the floor. This request will strengthen parental capacity by providing emotional and physical care. This request will lessen her stressors and free up some of her time to spend with her children ultimately enhance the potential for permanency. Any help, big or small, will bring her one step closer to providing a stable and loving home for her children. If you can contribute in any way, it would mean the world to this mother and her family. This family of four includes mom and 3 children (14, 9, 5). Family came to the attention of DCFS due to physical/emotional abuse from father. Mother has relocated herself and the children away from father. As a result, mother is experiencing financial hardship without father's assistance. Family lives in a 2 bedroom/1 bathroom home and they are in need of a sofa bed that seats three people. A sofa bed will provide a more comfortable living and sleeping space for the entire family, thereby increasing the safety and general well-being of the children and reducing stress and anxiety for mother allowing her to focus more on her family and reallocate her already limited resources to other needs for her children.

I am writing to request support on behalf of Mother, who is currently involved with our agency and actively working toward reunification with her children. Mother has successfully completed all court-ordered services and continues to demonstrate commitment to her personal growth and family well-being by regularly attending outpatient services.

Mother has recently secured stable housing and moved into her new apartment. As she works to rebuild a safe and nurturing environment for her family, she is in need of basic household essentials to help her establish a functional and supportive home setting for herself and her children.

The referred family is requesting assistance obtaining twin size bed for their child. The resource parent’s granddaughter is a 15-year-old with an intellectual disability. The resource parents care for her granddaughter on a permanent basis and is in the process of adopting her. The resource parent is attentive to her granddaughters needs and expresses her joy in caring for her. The resource parent is on a limited income and is an urgent need of a twin size bed for her granddaughter. The resource parent resides in a studio apartment with her granddaughter and is kindly request these items to continue proving a comfortable and adequate home environment. The family would appreciate any help and want to explore all the options to make the child as comfortable as possible at home.

I am writing to advocate on behalf of my client, who is in urgent need of steel-toed boots (women's size 10), 2 work pants size 6/8 size medium, and work gloves (large) in order to begin her newly secured warehouse job with expected start date of 7/08/2025. Currently, her available funds have gone to paying her rent for the month of July. These items are essential for meeting the safety and dress code requirements of her employer. My client is a single mother who is determined to maintain stable employment to provide for herself and her young son. Access to this job will offer financial stability and support her long-term goal of self-sufficiency. Without the required work gear, she risks losing this opportunity, which would negatively impact her progress and well-being.I strongly support her request for assistance with these items and urge any available resources or funding to be made accessible to help remove this barrier to employment. Client is a single parent of 4 minor children transitioning into permanent housing on 7/4/2025. Family has been homeless "couch surfing" for more than 2 years and recently was approved for an apartment. During their period of homelessness, family had to give up the majority of their belongings to move in temporarily in a friends small bedroom. Client is finally able to provide a stable living situation for her and children and is need of furniture: dressers, mattress, dining table and chairs, sofa or living room seating, basic kitchen supplies and cookware. Access to these items will not only improve the family's comfort and well-being but will also support their continued success in maintaining stable housing. Furniture can play a critical role in restoring a sense of normalcy, security, and dignity for individuals rebuilding after homelessness or housing insecurity. Dear Careportal, I am writing to respectfully request assistance in securing three beds and a crib for a single mother who is currently involved in an open DCFS case. This mother is a survivor of domestic violence and has recently taken a major step toward rebuilding her life by moving into her own apartment. Her children are in the process of being returned to her care, and while she is deeply committed to creating a safe and stable home, she currently does not have the financial means to provide beds for her children. Obtaining these beds would not only relieve significant stress for the mother but also offer her children a sense of stability, security, and comfort during this critical time of transition. Your support would allow this family to begin healing together in a safe and supportive environment. It would also enable the mother to focus on her continued progress, including maintaining her home and meeting the needs of her children. On 2/21/2024. Court will order and allow the child to be cremated. DCFS to make best efforts to pay for the cremation of the child.

DCFS Policy for Burial is only for Foster Care Providers only NOT parents and the fees have to be paid upfront, and forms submitted to be reimbursed. CSW reached out to the person who sent the Morgue Release Letter via email, with the Notifications Section LAC Medical Examiner-Coroner's Office. This CSW asked for the Health Department and County Morgue Tax ID numbers. This CSW was provided with a flyer on how to obtain the ashes. Obtain the Health Permit first. Can obtain the Death Certificate at the same time. Once fee is paid present permit obtained to the Medical Center and pay the Cremation fee. Once fee is paid, will provide the driving directions to the Los Angeles County Crematory/Cemetery. CSW informed that the County Morgue and Health Department do not have Tax Id's and therefore would not be able to submit for funding and no other funding available. Person would have to pay the fees upfront and get reimbursed. The Department is trying to assist Ms. Moore with obtaining Royal ashes however, there does not appear to be any funding available for this situation. Trying to obtain to obtain the ashes of the child, Royal Terry who passed away on 12/23/2023 from the LA County Morgue on behalf of Mother. Health Permit is $11.00 dollars. Death Certificate $24. dollars. Cremation $352.00 dollars. Total of $387.00 dollars.

I have a family who is struggling financially and are unable to get a crib for the 5 months old boy. The parents are dealing with unexpected expenses related to a very expensive surgery that the 5 years old needs to have as soon as possible due to a medical diagnosis. The parents are hopeful to be able to overcome the current struggles they are facing financially to be able to continue providing safe environment for the children to thrive. Currently the parents are co-sleeping with the 5-month-old, and are in need of a crib, mattress, and sheets so that the baby can sleep independently. The parents are not able to afford purchasing a crib at this time. Providing these basic necessities would diminish the emotional stressors on the family. By fulfilling this request, it would help the Department reduce the family's financial burden and help prevent court involvement and ensure family stability.

The mother is currently unemployed to be able to care for her six children. The only sole provider in the home is the father, and he work as a laborer. However, he has not been able to work much lately due to all the immigration enforcement currently going on in the city. The mother and father are also financially supporting the mother’s 16 years old daughter who has a 7-month-old baby of her own that also resides in the home. Currently, the mother and father are co-sleeping with the one-year-old child. The family is in need of a crib for the one-year-old to sleep independently. Providing these basic necessities would diminish the emotional stressors on the family. By fulfilling this request, it would help the Department reduce the family's financial burden and help prevent court involvement and ensure family stability.

The Department is removing an infant from the hospital and trying to place the baby with relatives. The baby is a preemie and is under 5lbs. The relatives were not prepared to have a baby in their home and will not be receiving any funding for the child for at least a month. In order to place the baby, the family is in need of a car seat, crib with mattress, and bedding. In order to help the family care for the infant properly, we are asking for a car seat with stroller and base travel system, crib with mattress, bedding, 3 packs of preemie/newborn diapers and wipes, and about 10 preemie/newborn outfits for a boy.
