Mom and Dad are trying to make ends meet while raising 5 children between the two of them. Mom has been trying to find a job but has also had vehicle trouble, so transportation is a hurdle for them. Their power was just restored and the family needs groceries. Mom says the family could use some meats, fruits for snacks for the kids, bottled water, canned items such as raviolis and spaghetti-o's and frozen pizza. One child was present during conversation and told Support Coordinator that he likes soft tacos, apples & macaroni and cheese. The family could greatly benefit from not only the groceries but a connection to a local church body.

This request is for a single Grandmother who has placement of 3 foster children in the home already and is taking placement of 2 more children on 5/23/2025. The bedrooms are small and after placing a 3rd bed in one of the rooms to accomodate the 2 new children there is no space to walk around in the bedroom. The request is for a set of bunk beds and mattresses. The caregiver has given permission for the KS to give Care Portal her information for them to reach out to her.

Single parent is currently on unsupervised visitation with 3 children. Parent is moving forward and going to be starting overnights into reunification as a single parent reunification. The parent has purchased 3 twin bed frames but is needing mattresses. These mattresses will provide for the children appropriate sleeping arrangements allowing for the parent to begin overnight visitation and be reunified.

Mom has just been reunified with the children. Mom is single and working two jobs to support herself and the kids. Mom has also been working on obtaining her GED to have the opportunity to attend college to further her education to be able to provide a better life for her children. Mom said the kids desperately need underwear and socks as there wasn't much returned upon reunification. Mom is very happy to have her children back home and the kids all appeared very happy to be back with mom. This request being fulfilled would greatly benefit the family and connecting with a local church would be a blessing for the whole family.

A single mother with 5 children is in need of 3 twin mattresses. Two of the children were recently reunited to her care. She has been working really hard to comply with everything needed in her case. She recently moved to this home and is trying to provide everything they need, but she can't afford buying the mattresses at this time. Mom is working but after paying rent and utilities there not much left to cover for the mattresses. She is transitioning between jobs and had a job interview yesterday. She wants to provide a safe place for her children and demonstrate that she is committed to keep her children on her care. Adequate sleep is important for children's brain development and support healthy emotional development as well. She doesn't mind barely used mattresses. She appreciates any help you could provide. The family refrigerator went out a few weeks ago which is causing a gnat issue.

The mother is a single mother who works from home while residing with maternal aunt and other children in the home.

The relatives are not part of the case. The mother is doing the best she can. The refrigerator going out was an

unexpected cost for the mother, and she does not have the money as she is the sole provider for the children. She

needs the refrigerator to ensure the children have adequate food in the home at all times.

She and her five children recently relocated due to the violence between the mother and father. The mother lost her food stamps last month for her and her five girls due to the father claiming the girls to get his own food stamps. The mother had to provide proof she has custody of the children, and not the father. She works as a bus driver. The mother reports that she decided to ensure her children were fed so she can’t pay rent this month. She will be getting her food stamps back and will be able to catch up. The mother reports that every 4 days the fee goes up $150. She has been given a notice to evict if she does not pay. Her and the children have no place to go if they lose the home.

Two children are placed here with relative from Indiana. The family recently lost their car. The children go to school out of district due to School Choice program. The relative has no way to get the children to school without a car. She has a rental car through tomorrow, but can't afford it after that. We are requesting transportation assistance to help these kids get to and from school until the end of the school year. Any assistance would be greatly appreciated. I'm not sure what to put for exact need. They need a vehicle but can't afford one right now. But the main issue is getting the kids to and from school.

We have had a single woman take in two sweet little boys and she needs a little help. She is doing everything she can but any help would be a blessing. She is asking for help with Pull-ups, Diapers, and summer clothing. Pull-ups: Size 4/5T Diapers: Size 4 Summer Clothing: Sizes 4T and 2

Due to medical reasons a FP is asking for some time off. We are moving the child to another NYAP home and the new home needs a trip bunk bed to accommodate this child. The FP already has three children in her home. This will be a 4th placement. She is moving to a larger (3 bedroom) apartment and needs to furnish the room with a bed that can hold the children. One set of twin bunks are needed

The Foster Parent has stated that regular bunk will work. We are requesting one set of bunk bed frames and the mattresses.

This single mother has just been reunified with her 2-year-old son. Mom has been working hard to keep a job and a home for herself and her boy. She has been cooperating and making all the needed changes to be reunited with her child. Mom is requesting assistance with acquiring a convertible car seat for her son because he is quickly growing out of the one that she has for him. Mom wants to make sure that his car seat meets legal standards for his size, and to make sure her son fits comfortably in his car seat. This will help the family get to appointments and daycare safely.

Mom and the son are living in dad's apartment. The child has special needs and also suffers from a vomiting disorder. Mom is asking for help with a L shaped couch because it will fit their space best, one that has easy to clean material due to the vomiting disorder. Mom is also requesting help with a twin bed or futon to put into the child's room so that she can sleep in the room with him on rough nights and just spend more time in his room that she has fixed up as a safe space for him with padded floor mats, his bed, developmentally appropriate toys for him and characters that he likes. Mom also stated that a changing table (Various Baby Supplies) would be a great help.

This family is still in need of an L shaped couch that is easy to clean, due to a condition that the child has that causes him to vomit often. The family is also still in need of a changing table for the child.
