Mother is receiving family reunification services through the Department of Children and Family Services. Mother has made substantial progress in addressing her issues and has complied with all of DCFS and Court ordered treatment plan. However, mother does not have stable housing. She reports she is temporarily staying with different friends but mother states their home is not able to accommodate her elementary and middle school age children. Mother does not have the adequate housing nor the resources to secure alternate housing to provide to her children so that she may safely care for them, if children are return to her care. Mother is undocumented. Court has granted overnight visits and the visits may take place in a hotel. The mother is asking if anyone can provide her with a gift card to be use to pay for a hotel. Mother has to show that she is able to care for her children during her visits. Mother would greatly appreciated and the children.

The family is currently in dire need of gift cards (food4less or target) and a stroller for the one-month-old. Mother recently separated from the father of her youngest child who was their sole provider. In addition, mother is in need of a stroller for the infant and the purchase of a prepaid phone, as she was on a plan with father who is no longer in the home and providing support to the family.

I am writing to seek assistance of a caregiver- a grandfather who is currently the primary guardian for his 16-year-old grandson. This responsible grandfather has stepped into a vital role, providing emotional, physical and financial support to his grandson during a critical period in the young man’s life. The grandfather is on a fixed income and faces financial strain, particularly as he also pays out-of-pocket for essential medical expenses. The grandfather is seeking assistance to obtain two sets of sofas to help furnish the home for both comfort and functionality. Providing the two sets of sofas would help the family remove the old ones and improve the quality of life.

Please help strengthen a biofamily led by the mother. The mother was a victim of severe domestic violence where she felt trapped in her own home. Mother was spied on and threatened that if she disclosed the domestic violence that she was enduring, she and the children would be killed. When mother found the opportunity to leave that situation, she had to do it promptly and without any preparation. Mother and her three children left the home with only the clothes on their back. The sudden decision was made as mother believed it was the only way to save her self and the children.

This family would really benefit from some gift cards for clothing for herself.

We are asking for case of diapers and wipes as well as gift cards for groceries for a mother who has reunited with her children but is financially struggling at the moment. Meeting this need would help immensely with mother's mental health and stress. Mother has completed all her programs and therapy.

A counselor at a local high school shared a need for a local family. The family was displaced, and did seek a motel voucher from the district. Thankfully they're back in an apartment, but the mom shared they're in need of a stove.The oldest a senior at the high school and there are two younger brothers. The family is requesting help in getting a gas stove.

We are asking for support for this family of a crib for their three month old infant. The baby has been sleeping with her parents and mother was told this is considered a health risk. At this time the family has no extra funds. Mother does not work and father is out of work on disability. This crib would support this family in staying unified by eliminating the immediate concern of where the infant sleeps.

I looked at Target and they have a crib that also converts for the child as they age to a lower position preventing the child from crawling over the edge. I have included the details below.

NMD has maintained steady employment for years and now works with a large tech company. She has been a very respectful, responsible, communicative, independent, and a resilient young adult. This assistance will further her ability to become self-sufficient as she is well deserving of the help with her success in maintaining compliance with the department and should be rewarded for her great works and commitment to not only remaining eligible for services, but most of all progress for herself. She is in need of assistance in her transition out of her current home and into assisted living. She is in need of shirts, pants shoes and hygiene products.

I am working with a family of six—a mother and her five young children—who are in need of support. Family has experienced homelessness and has been able to overcome many challenges. As the children have grown, the limited space in their home has made it difficult to provide each child with a safe and comfortable place to sleep. To help meet her family’s needs, the mother is seeking assistance with obtaining a bunk bed.

This furniture would significantly improve the children’s living conditions by creating more space and ensuring a safer sleep environment. Due to ongoing financial hardship, the mother is unable to afford this on her own. Providing this support would strengthen her ability to care for her children and offer her greater peace of mind as she continues to meet their daily needs. Client is 27 year old mother. Client has successfully enrolled in Welcome Baby and is an active participant. Client has 4 children ages 5, 4, 1, and a 3 month old. Upon assessing needs, client is in need of bunk beds for her 5 and 4 year olds. Client stays home to care for her 4 children. Client is not able to work due to her 5 year old having health issues and her 1 year old having Down Syndrome. Client and family would benefit from the bunk beds so they can sleep independently and safely. Client would greatly appreciate receiving assistance so the children can sleep safely in their own space. Thank You in advance for your support and consideration with helping this family. Mother and children were homeless, living in the motel room. Mother was able to put some money together to rent a one-bedroom apartment in El Monte. The apartment does not a refrigerator, stove or even a microwave, needed to provide food for her children. She is a single mother to young children and is looking to better her life for her children's sake. Mother is complying with Court orders. Father no longer resides with the family and has monitor visit with the children.
