This mom has a beautiful faith and has chosen life for her baby despite difficult circumstances and lack of support. She has worked two jobs - 7 days/week in order to provide for her and her son. Despite working so hard, she is struggling to pay for all of her normal cost of living after she has paid her housing and car bills. She makes just enough not to qualify for public assistance. Gas and grocery gift cards would help her greatly.

Child, age 15, came into DCS custody after his mother passed away. He was the one who called 911 and she his mother pass away in front of him. Mother passed away from an overdose. The child's father has not been involved in his life much, and the child does not care to have his father in his life as they do have a rough relationship. Child currently lives with his adult half brother. A few weeks ago, they moved out of the house he was originally placed in due to half-brother's father not wanting the child there. They moved in to their half-sister's home and were only there for less than two weeks, and had to move out due to certain issues. After this, they moved in to an extended stay for a few weeks and just a couple of days ago got in to an apartment. They would like to request some items for their new home they can finally make there's. Thank you!

The household is Mother, Step-Father, and her 5 young-aged children (9, 8, 6, 5, 3). Mother stated the family recently moved away from an unsafe area/living situation and were not able to take many of their belongings with them. The Mother is looking to get beds for the family as they are currently sleeping on the floor or on pillows. The family is looking to get 4 twin-size beds and 1 queen-size bed. There is no preference on anything and they would appreciate anything they can get.

The family has 10 children, ages newborn through 17 years old. They have recently moved in with their mother. The mother was homeless for a couple of months and was receiving resources to help her obtain housing. Once she was able to find a home she had all of the children move in with her, due to their father having to move out of the place they were residing. The parents are separated because the mother found out that the father was using substances while he was at work and no longer wanted to be with him for lying to her. The mother had been sober of substance abuse for more than 10 years and expressed an interest in completing her GED to receive a higher income to provide for her children. The mother only has two twin mattresses that are pushed together and a couch that allow the children to sleep. The mother is in need of mattress for all of the children (a crib frame/crib mattress, 3 toddler mattresses/ bed frames, 6 twin mattresses/bed frames). When the mother moved into the home, she does not have a lot of items and expressed need of cleaning supplies, blankets, and kitchen sets of bowls, plates, and silverware. If the family is able to receive any donations, they can continue to settle into their new home and make the bedrooms special places for the children to sleep in there comfortably.

The household is Mother and her 2 children. The family recently moved here from Ohio and are trying to get items for their home. The Mother is looking to get a full size bed for the children and some bedding. The children are 6 and 7. There is no preference on anything.

This client is expecting a new baby boy in a few months and currently has nothing for baby. She lives with her husband and daughter. Her husband is working but finances are really tight for the family.

The mother just relocated from North Carolina to Phoenix, Arizona, with her sister. She was a victim of a domestic violence dispute. She was able to get away, obtained an order of protection, and relocated to Arizona to ensure her and her child's safety. Since she had to travel across state lines, she has very minimal baby supplies and is in need of more. She will also be referred by the Department to other community services since she is new to Arizona. The mother traveled and was not able to bring only limited items. She needs a bassinet, baby bottles, diapers size 4, wipes, she would like a car seat and clothing is a size 12 months.

This is a single mother to 2 children under the age of 18. One of her children is in school and doing extremely well. Her other child is in school as well and is diagnosed with low functioning autism. He is 10 years old and functions at about 2 years old. This child has been escaping their home, on and off for the past several years. The family has taken several protective measures to keep this from happening, but recently happened again. The family has a double locking deadbolt on their door, needing a key to enter and exit their home. The child will find where the key is hidden, and unlock the door causing him to wander from the home. The child enjoys going swimming, and typically ends up near a pool, however is unable to swim on his own raising another concern. The mother has worked extremely hard to keep her child safe, and having a combination code safe that she is able to fit all of the keys into, and change the combination to is something that could keep her child safe.

This is a single mother to 4 children under the age of 18. She has a child that is diagnosed with autism, as well as another child who is being assessed for autism. Over this past weekend, her 5 year old child escaped from her home at approximately 12AM, when the family was sleeping and was not found until several hours later by a stranger. The family is needing assistance to strengthen their bio family by these needs. The mother is very open and honest about the assistance she is needing and is extremely heartfelt and remorseful. The mother has had a difficult time keeping up with her home due to her children playing, and is in need of basic cleaning supplies. She is accepting to any and all help, and would enjoy having carpet cleaner. She is also in need of more safety measures to prevent her child from escaping her home again, a chain lock as well as child safety circular handle door knobs would be of assistance to her family. Grandmother took in her two granddaughters - ages 11 and 13. Only the 11 year-old resides at home with grandma. There are a couple doors, a window, and holes in the wall that need repairing. Grandma is in a wheelchair and only works part-time. She takes care of her granddaughter's needs and is very strong in her faith. This would greatly help grandma, as her income is limited and there are other bills she currently has to pay.

Mother just got her apartment and is in need of new furniture and kitchen supplies! Mother is asking for all the help that she can get to make the house feel more like a home and when she is reunified with her child, she wants to make sure the home is all set up and made to feel comfortable.

Currently, the family is just moving into their new apartment. The family was needing housing to finally start the reunification process with their children. The family also has a new baby that was not brought into care. Currently, 3 of the children are placed in foster care.
