Client is a single mom of two and is currently in need of a stroller for her 19-month-old son. Client explained he is quickly outgrowing his old stroller as he is 25 pounds, and is in need of one that can grow with him. Client would like a 2 in 1 convertible toddler stroller, preferably one that is also collapsable for being on the go. Client would greatly benefit from a new stroller as she is currently balancing work, school, and parenting; and would have one of her biggest needs met by receiving a stroller.

Per First Care policies, equipment items must be brand new

My client is a young mother of 2 children. One of the children turned 1 in March while the other is only 4 months old (both girls). The young mother is working hard to utilize services and support to be able to provide for her two children. The young mother has dealt with domestic violence in the past and is working to stabilize herself and her children. My client has signed up for benefits and was denied due to unavoidable circumstances. My client is in need of basic needs for the home and for the wellbeing of her two daughters. The groceries: Rice, Vegetable oil, Chicken, beef, 100% fruit juice, sugar, chef Boyar mini micro beef ravioli, yogurt, protein shake, Pedicure, fruit pouches, yogurt for toddler, mealtime for baby power blend vegetable chicken, stage 1 Gerber, snacks for toddlers (no pork or gelatin) mealtime for toddler pasta sauce and meat stars, capri sun.

This family has experienced housing instability as well as food insecurity for much of this school year. Mom's rental assistance just decreased drastically and now she is struggling to keep up. She will be getting a new job soon and is currently training for it, but is currently getting behind and struggling to catch up. She indicated that this is causing her to struggle to afford other household basics as well as food for her family. With summer coming up and there no longer being 2 meals provided at school, this is a major concern.

She does not have a car so the best grocery option would be a delivery of specific items or a Cub gift card.

Spanish speaking. This family of 6 recently immigrated to the US, they have 3 minor children (all boys, ages 2, 11, and 15) and one 18-year-old son. They moved into a new place and need some household items to feel more settled. They do not have very much furniture and need a dining room table and prefer one that is like a folding card table with 4 chairs for the children to do their homework and eat meals. They need a sofa couch and bedding for their full-sized bed. The bedding needed is a protective waterproof bed cover, full-sized sheet set. This will provide a better sense of well-being, which is very important for their culture and this family. The family resides in a single family home. There are cement steps to get into the home.

Client is a single mother with a 13-month-old child. Client is in need of a new stroller for her child as he is quickly outgrowing his current one. Client is struggling financially, and would greatly benefit from receiving a stroller so she can use her income to cover basic necessities and be able to take her child around more freely. Client is specifically wanting a convertible baby stroller that is reversible and foldable and can grow with her child.

In need of a bunk bed to help the father reunify with his children. The children are two young children who love their father and want to be with him. He struggles with addiction however, he has obtained sobriety over the past two months which is a huge success for him. Any help would be appreciated.

This family is new to MPS and is experiencing housing instability. This lack of stability is impacting her children as well as her comfort and safety.

As a single mom, she struggles to afford groceries. This is a major concern especially with summer coming up. Any gift card (or grocery dropoff) would be appreciated (Cub, Walmart, Target).

She is also looking for cleaning supplies: Laundry detergent, Tissues, Trash bags, Toilet paper, Paper towels, Dish soap.

Clothing needed: Socks, underwear, t-shirts/undershirts, shorts, shoes - Boy age 5: Size 6/7, 13/1 shoes; Girl age 3: Size 3t, 9c shoes; Boy age 2: Size 2t, 7c shoes

Mom is a single parent to 5 boys, two of which are high-need special education students. Recently, she had $7,000 dollars stolen from her by someone who she really trusted. Now, she is in a crisis finding a way to keep things together. She was just about to finish school, but now is struggling to even attend due to all of the stress on her right now. Items needed:

Cleaning Supplies (Paper towels, disinfectant wipes, PineSol, dish soap, hand soap, bathroom cleaning products, sponges, Lysol spray);

Household items (Silverware, cups, bowls, plates);

Hygiene items (Deodorant, shampoo, soap, conditioner, lotion); Toilet Paper;

Boys' Clothing (10 year old - size 10/12; 7 year old - size 8; 6 year old - size 7; 3 year old - size 4T; 18 month old) -

Clothing needed: shorts, pants, t-shirts, underwear, socks.

All boys favorite colors are blue, green, and yellow.

This student's family is at risk of losing their housing because of increasing prices and Grandma (guardian) losing her job. She is struggling to get unemployment or any other support. She works incredibly hard to take care of her grandson and who has a high level of needs. As the head of household for a house of 6 people, she does a huge amount of work.

Grandma just finished school and is hoping to find a job soon, but right now things are very difficult. The housing instability is causing our student increased stress during the school day, which is greatly impacting his ability to control his emotions at school and learn. He has gone through a huge amount of trauma in his life, so any additional stressors cause him to become very disregulated.

Household members: Grandma, Great-grandfather, 13 year old, 20 year old, 8 year old (our student), 11 month old.

I gave grandma a financial assistance resource, but support in other ways would be incredibly appreciated to help this student be more successful in school and for the family to maintain their housing.

Grandma indicates the major need is assistance with food. Feeding a family of 6 is very expensive. Any gift card or delivery of groceries would be appreciated. (Walmart, Cub, etc). Thank you.

This is a two-parent household with five children under the ages 6 and under. There is one child with specialized needs and other children who are participating in the evaluation process to assess what additional needs they might have. The father of the children has had a loss in his family and had to travel out of the country to address this loss, and this put additional strain on the household due to loss of support for the family both physically and financially. In addition, their youngest child is an infant therefore the mother of the children has only recently been able to return to work to help provide for the family financially. These children would benefit from the following support to enhance their learning, development, and safety.

There was not an option for a crib mattress protector, so I had to enter it as twin, but we need a crib size for the baby. Due to space if the dresser could be a taller type rather than the shorter/long it might work better in the family's home.

A 16-year-old female teenager has been running away from her family for years and has finally found stability with an adult family friend who will be caring for her until she becomes an adult. She has been with the adult family friend for a couple of months now and is doing really well. The caregiver has 3 kids of her own and another teenager that she is also caring for in the home. The caregiver has been sleeping on the couch so the teenager can have her bed. The family is hoping for a leather futon so the teen can have a place of her own to sleep. The apartment is small for all 6 people to be living there, so they are looking for new housing; a futon will fit in the current apartment. The family is wanting a leather futon due to having prior experience with bed bugs in a previous living situation that has left them fearful of fabric furniture. They are open to a fabric futon if it is completely new.

Client is in need of a play mat/activity pad for her daughter who just turned one month old. Client has had some support from family but is not working and is mostly home alone with her baby. The play mat/activity pad would greatly improve baby's wellbeing as she is growing and developing, and allow client to prioritize making other necessary purchases.
