Case Manager works with a single mom who is parenting their child who has an autism diagnosis. Child has been refusing to go to school which has caused mom to have to use FMLA and she is now at risk of losing her job. Looking for help with rewards due to her limited income now. Rewards for behavior program to attend school for child with autism. He loves skittles, m&m's, Hershey's plan small bars, nerds gummies berry flavor mainly, zero sugar root-beer, sprite or 7 up in small cans like 4oz. Fruit snacks or small/snack bags of chips (plan potatoes chips, plain pringles, Cheetos, or regular Doritos. Also, he loves bubbles pretty much anything having to do with making bubbles...

This mom has had a series of medical issues throughout her pregnancy & some have persisted into the postpartum period, including vertigo & migraines. Baby was born early at 33 weeks and she was in the NICU for 6 weeks. Mom is alone with one older cousin in town. Mom divorced her husband when he took another wife. She has minimal furniture/infant supplies. A crib & bed were donated. But she only has 1 hard desk chair. It would be so helpful if mom had a comfortable rocking chair to feed/rock/cuddle baby in. This would mean so much to mom.

This family has been struggling to make ends meet. They are living on one income as the other provider in the home has been laid off work and they are unable to meet the needs to adequately support the children, especially as they begin to grow out of old items. Having the items listed below will increase the children's safety and wellbeing. I am working with a family that consists of a mom, dad and 4 children, mom is also pregnant and due later this year. During the last few months, the family has been able to secure employment (both parents), secure housing, the children have started daycare, and they have found ways to repair their vehicle. The children’s maternal grandma has just moved into the home and is not able to move around well due to previously being in a nursing home. The family is looking for an electric recliner to help ease their grandma’s stay and for her to be able to be comfortable in the home. The family has been able to utilize other resources and family members to get other furniture in the home but are still looking at getting this recliner for the grandma. Any help is appreciated if able.

I am working with a single mom with a newborn. She has had some issues with vertigo and headaches. She often has family come and help her with her newborn and they don't have anywhere to sit. She is looking for a couch.

I am working with a family with three children (ages 14, 8, 1). The father is currently incarcerated, and the mother is having to pack up the family home as she is no longer able to stay there due to being unable to afford the rent. The mother is needing help with cardboard boxes and totes and things that she can use to pack up their belongings in. The mother is having to pack up the whole house so is needing a quite of bit of moving supplies. The family is currently staying with the maternal aunt and plan to stay there until they are able to find a place of their own. The family appreciates any help that it can get.

This request is for a sofa set, couch and love seat, for new family to this community. The family is working hard to improve their financial stability, but they are still struggling with many barriers.

Mom and 3 kiddos. Mom does not have a car and would like to walk during nice weather with her 3 kiddos. Mom has a 2 year old and a set of twins that are under 2 years old. She would like to sign them up for daycare and would like to walk to the daycare with them.

Mom and 3 kids. 1 set of twins and a 2 year old. We need a toddler bed for the 2 year old. Mom is struggling with making ends meet and needs help with getting a toddler bed.

Family is in need of dishwasher as theirs recently broke and they do not have the funds to fix or replace.

Family has moved from Chicago to be with mother/grandmother. Pervious housing was two bedroom apartment. They have secured new housing but need help to furnish the rooms of the home that now meets the family needs. Some of children do not have a dresser or bed.

Most important needs: -2 couches -a mattress (either queen or full size) -2 end tables -2 dressers

Other needs of possible: -TV -Lawn mower
